- An active Texas Physical Therapist Assistant license in good standing is required.
- The candidate must demonstrate clinical judgment and work independently in the field within PTA scope and under PT supervision.
- The candidate must be able to escalate changes in condition to the physical therapist, registered nurse, physician, or medical doctor and document the rationale clearly.
- The candidate must have strong communication and documentation abilities.
- The candidate must reliably follow treatment plans, visit cadence, and patient goals.
- A current driver's license and comfort with daily travel across the assigned territory are required.
- The candidate must be able to carry or transport basic therapy equipment as needed.
- The candidate must be competent in core home health physical therapist assistant scope, including transfers, gait training, balance and fall prevention, assistive device education, strengthening, functional mobility, home safety assessment and modification education, and patient/caregiver coaching with practical home programs.
- Deliver skilled physical therapist assistant visits in patients' homes under the supervising physical therapist's plan of care.
- Execute treatment plans focused on functional mobility and measurable progress, including gait and balance, transfers, strengthening, home safety, and fall prevention.
- Set and track objective progress within physical therapist assistant scope and communicate updates and concerns to the supervising physical therapist.
- Coordinate multidisciplinary care by escalating changes in condition, aligning on orders, and keeping the care team moving.
- Coach patients and caregivers on practical home programs.
- Document promptly and thoroughly in Homecare Homebase.
- Prior home health experience is preferred.
- Experience with Homecare Homebase or a similar home health electronic medical record is preferred.
- Deeper clinical experience with orthopedic or neurological populations is preferred.
- Strong organization and time management are preferred.
