Job Description:
The clinical case manager supports the functioning of the Student Counseling Center by facilitating access to mental health care for students that is beyond the scope of services available on campus (i.e. facilitation of community mental health referrals for students needing long-term or specialized mental health care; coordination of care upon discharge from hospital when inpatient psychiatric care is warranted). The clinical case manager is involved in direct clinical service to students by participating in triage intakes, crisis intervention, and by providing intensive case management and transition services (bridge care) to facilitate stabilization until adequate community care plan is established.
Essential Job Functions:
30 % A. Support clinical flow within student counseling center by providing direct 1:1 clinical services to the student population, participating in scheduling, triage intakes, crisis intervention, wait-list management, and providing bridge care when needed, coordinating hospitalizations initiated by center clinician, and timely/ethical management of associated documentation.
20 % B. Provide clinical case management for students warranting higher level of care to (1) act as liaison with treatment team to ensure adequate discharge/step-down care of students receiving inpatient or intensive outpatient psychiatric care and (2) act as liaison to relevant campus offices so they can best facilitate any warranted campus supports and (3) to evaluate adequacy of discharge plan and return to campus paperwork (4) communicate with collateral parties when appropriate and with student’s consent,(5) maintain timely, accurate documentation.
10 % C. Serve as senior staff member on call during business hours to aid in high level campus emergencies, hospitalizations and emergency petitions, and routine consultation with clinical trainees as needed.
10 % D. Participate in intra-office meetings and responsibilities to support overall functioning of the Student Counseling Center including development and review of relevant policy/procedure sets, contribution to continuous quality improvement initiatives that are data-driven and in alignment with best practice standards, staff meetings, case conference, trainings, mentoring of graduate clinical externs.
10 % E. Serve as community referral coordinator by (1) maintaining referral and resource database, establishing relationships with key referral sources and maintaining knowledge of current wait times, scope of services including capacity to support students from diverse backgrounds, insurance, area of expertise, establishing and maintaining MOUs with local hospitals and mental health resources as appropriate; and (2) working directly with students, prospective students, and families to facilitate referral and establishment of care with a community provider.
5 % F. Participate on relevant campus committees and/or teams that have the mission of promoting health and wellness in regard to mental health, substance abuse, and/or sexual assault and relationship violence.
5 % G. Provide consultation and outreach services to students, campus and community members related to mental health issues and student concerns.
5 % H. Ongoing professional development including obtaining professional consultation when needed, staying abreast of standard of practice trends in the literature, active membership in relevant state/national professional organizations and maintenance of ongoing consultations/collaborations with peers in parallel positions at peer-institutions, and participation in continuing education.
Education:
Required: Master’s degree in mental health field (social work, clinical/counseling psychology) required for terminal licensure within 6 months of being hired.
Professional Experience:
Required: 1 year of experience in clinical case management; Preferred: Some experience in college mental health, including crisis assessment and brief intervention.
