Professional Case Management expands outreach leadership as federal home healthcare demand grows.

Professional Case Management expands outreach leadership as federal home healthcare demand grows. May 19, 2026 Voula Siderakis Professional Case Management has appointed healthcare sales and operations executive Voula Siderakis as vice president of outreach for its home healthcare division as the company expands engagement efforts tied to federal benefits programs serving former nuclear weapons and uranium workers. Siderakis will oversee outreach and business development initiatives across PCM's 17 regional offices, with responsibilities focused on expanding patient access and strengthening referral and advocacy partnerships connected to the Energy Employees Occupational Illness Compensation Program Act, or EEOICPA. The appointment comes as home healthcare providers continue scaling operations to meet growing demand for in-home care services tied to aging populations, chronic illness management and federally supported care programs. Providers operating in specialized reimbursement environments such as EEOICPA face additional operational complexity involving eligibility coordination, patient advocacy and multi-state care delivery infrastructure. PCM specializes in serving former nuclear weapons and uranium workers who developed occupational illnesses connected to work in the energy sector. The company provides in-home nursing, caregiving and long-term support services funded through the federal compensation program. President Greg Austin said Siderakis' background in healthcare outreach and client advocacy aligns with the company's strategy to broaden patient engagement and improve access to home-based care services. Siderakis most recently served as regional sales director at BAYADA Home Health Care, where her responsibilities included referral network development, operational productivity improvement and revenue growth initiatives across regulated healthcare environments. Her appointment reflects the increasing operational importance of referral management, provider partnerships and benefits-navigation support in the home healthcare sector, particularly for organizations serving specialized patient populations tied to government healthcare programs. Home healthcare companies have increasingly focused on expanding localized outreach infrastructure as competition intensifies for patients, caregivers and referral relationships. Building strong partnerships with healthcare systems, case managers and advocacy organizations has become central to sustaining patient acquisition and continuity of care. PCM said Siderakis will help drive partnerships with healthcare providers and benefits advocacy networks nationwide as the company expands its outreach strategy. The broader home healthcare industry continues shifting toward more decentralized care delivery models as payers and healthcare systems attempt to reduce institutional care costs and improve patient outcomes through home-based treatment and support services. For providers serving federal compensation programs, operational scale also depends heavily on navigating regulatory requirements, coordinating documentation and maintaining geographically distributed caregiver networks capable of delivering consistent in-home care. Professional Case Management positions itself as a specialized home care provider focused on patients connected to the energy industry workforce, offering services ranging from periodic support visits to continuous around-the-clock care designed to help patients remain safely in their homes.

BAYADA the only home care provider to win Modern Healthcare's 2026 Innovators award for its HICU(R) program.

BAYADA the only home care provider to win Modern Healthcare's 2026 Innovators award for its HICU(R) program. PHILADELPHIA, PA - April 13, 2026 - BAYADA Home Health Care, the nation's largest independent nonprofit home health care provider, has been named to Modern Healthcare's list of 2026 Innovators. Notably, BAYADA is the only home care organization recognized, distinguishing it among hospitals, tech giants, and health systems. The award recognizes BAYADA's Home Intensive Care Unit (HICU(R) program - a first-of-its-kind model that bridges the precarious gap between neonatal/pediatric intensive care (NICU/PICU) discharge and traditional home nursing. Redefining the Transition to Home: A Distinct Clinical Model For payers and hospital systems, the "stuck" patient is a chronic challenge. Medically fragile children often remain in ICU beds long after they are clinically stable because the home-based infrastructure to support them doesn't exist. BAYADA's HICU program is a 12-week, stepdown home care solution specifically designed to solve this. It is the only program developed by a home care company that: * Deploys critical-care trained pediatric nurses with 24/7 coverage at the start. * Safely tapers services as the child stabilizes and the family gains clinical confidence. * Integrates high-touch care coordination before and after discharge, including interdisciplinary rounds, seamless equipment setup, and 24/7 clinical backup. Quantifiable Results for Payers and Health System Partners The HICU model is both a clinical success and a financial imperative. By moving complex nursing care safely into the home setting, BAYADA has delivered measurable impact since the launch of the program. HICU outcomes to-date: * 83 children transitioned home safely * 6,307 hospital days avoided * 25% reduction in hospital readmission rate * $250,000 average cost savings per patient per year * 97.6% reliability rate The Human Impact: Drew's Story Beyond the data, the HICU program offers families a lifeline. Andrea Smith's son, Drew, was born preterm at 24 weeks and was dependent on a ventilator. While other providers couldn't offer the necessary level of home care in their area, the HICU program stepped in to help bring him home from the hospital. During his 12 weeks of HICU support, Drew avoided 84 days in the hospital, saving $388,000 in total health care costs, all while meeting every developmental milestone in the comfort of his home in the Philadelphia area. "He has never been back to the hospital - no hiccups, no ER visits," said Andrea. "They saved my baby." Advancing a National Culture of Innovation Modern Healthcare's recognition underscores BAYADA's commitment to innovation to bridge systemic gaps. Rather than relying on static care models, BAYADA has convened a multidisciplinary advisory council of hospital leaders and parents who help to regularly improve its HICU care delivery model. In partnership with Lehigh University, this research-backed model is now scaling nationally. BAYADA is currently expanding its footprint in the Boston area, Virginia, and Massachusetts to support hospital and payer partners looking to raise the standard for pediatric home care. Bayada Rolls Out New AI Model To Prevent Falls, Reduce Hospitalizations

Bayada rolls out New AI model to prevent falls, reduce hospitalizations. Bayada Home Health Care has recently rolled out an AI model to prevent falls and reduce hospitalizations among its personal care clients. Called the Enhanced Quality of Care Model (EQoC), the model assesses clients' risk levels and enables Bayada to make faster interventions, improving outcomes and setting the stage for more successful negotiations with payers. "By having that real-time information, we're able to be predictive instead of just reactive," Matthew Kroll, Bayada's practice president, personal care services, told Home Health Care News. "To paint a picture, we've always had fantastic caregivers who would call us and say, 'Something's wrong with Mrs. Smith today. I'm just not sure what's going on.' Then our nurse would talk to Mrs. Smith or the family, etc. We're not reliant on that phone call; now we can actually see in the data what's happening." When Bayada first moved from paper to electronic documentation, the company realized it had a treasure trove of unused information from its nurses and caregivers in its practice management system. On top of this, Bayada had data from another system used to document incidents, such as falls. "Anytime a client falls, anytime a client's hospitalized, anytime anything bad happens, we document that in an incident report," Kroll said. "We said, what if we took those two data sets and we combined them." Bayada provides home health, home care, hospice and behavioral health care services in 23 states. The company also has international locations, operating in Germany, India, Ireland, New Zealand and South Korea. Bayada has about 32,000 employees nationwide. On Thursday, the nonprofit announced its next CEO: the former president of Carelon Health, Bryony Winn. The company zeroed in on fall risks because they are the biggest predictor of hospitalizations among people over 65. While falls are a priority for Bayada, Kroll explained that EQoC has over 40 detection points. The decision to combine the two data sets was the beginning stages of Bayada's model EQoC. "We looked at all the patients who had adverse events like hospitalizations or falls with major injury, and we looked at all of the care documentation," Kroll said. "Could we find any patterns in the documentation that might signal to us that a fall is coming... so we did that analysis, using large language models. We found, essentially, it wasn't any one thing. It was a combination of things that were changing a client's risk for an adverse event." Bayada took all of this information and began scoring all of their clients on a risk profile - high, medium or low risk. "Our home health aides document information every day," Kroll said. "Did they give the client a bath? Did they support them with meals? Our schedulers are putting information in. All of that is going into a large language model that updates a risk dashboard every single day based on what's happening. If a client's risk changes, our nurse reaches out to them, sometimes in person, sometimes over the phone, and will offer different interventions." These interventions include physical therapy, additional care hours, home health services and prescription management, among other services. When implementing this model, Bayada decided to focus on its personal care segment because these clients typically receive care for a longer period. Kroll noted that instead of serving clients for 30 to 60 days, some individuals have received care from the company for years. "We wanted to focus on [if we] could see risk coming with this particular population, which generally is a population that we have the least amount of data on, and the least amount of tools have been built specifically for this population," Kroll said. "That's why we wanted to go out and build our own tool here. We're just now starting to see others come into this space, but much more effort and energy [have] been put [into] the home health and hospice environments." Eventually, the principles behind Bayada's fall prediction model can be extrapolated to the rest of the organization's offerings, Mike Johnson, chief researcher of home care innovation at Bayada, said on a recent episode of HHCN's podcast. Data demonstrating the benefits of this model can then be leveraged in payer negotiations. "When you talk to Medicaid payers who are... under a ton of stress, if we can show them a way that we can be proactive and reduce readmissions or admissions, not just readmissions, that's a conversation that they're interested in," Johnson said. Bayada is not the only provider leveraging AI predictions to improve outcomes. Team Select Home Care recently launched a predictive analytics platform specifically for its pediatric respiratory patients. While the platform is still early in its deployment, the organization has already seen a "significant reduction" in hospitalizations.
