Requirements
  • Graduate of an accredited school of nursing.
  • Current license as a Registered Nurse in the state of Massachusetts.
  • Current CPR and Alzheimer’s certification, or willingness to become certified within 60 days of hire, is essential.
  • At least two years of recent experience providing direct care to adults or chronically disabled persons, including at least one year caring for a frail or elderly population.
  • Access to reliable transportation to perform work throughout the PACE center’s service area within a 30-mile radius when needed.
  • Willingness to occasionally assist other Summit ElderCare PACE centers in person or remotely when there is an opening or a gap in coverage.
Responsibilities
  • Review and implement providers’ orders.
  • Conduct nursing assessments according to policy guidelines, including physical, psychosocial, behavioral, and MDS-HC assessments as indicated.
  • Involve participants and significant others based on their needs and abilities.
  • Deliver skilled nursing services to participants in the home setting according to individualized needs and age-appropriate nursing standards.
  • Provide for participants’ cultural and diverse needs when delivering care.
  • Monitor and evaluate therapeutic interventions.
  • Participate in the development and ongoing review of each participant’s care plan.
  • Delegate or accept participants’ care responsibilities to other members of the healthcare team when appropriate.
  • Identify emergency situations and initiate appropriate nursing orders and interventions.
  • Meet participants’ needs in a timely manner.
  • Participate in the interdisciplinary team process and collaborate with team members to meet participants’ needs.
  • Document all aspects of participant care, including significant health-status changes, monthly nurses’ notes, and healthcare teaching, in the medical record.
  • Participate in the training and orientation of new nursing staff as assigned.
  • Assist in delivering other nursing services as assigned.
  • Participate in the nursing on-call rotation as assigned.
  • Initiate and complete falls, infection, grievance, and incident reports.
  • Participate actively in site-specific committees.
  • Participate in the post-interdisciplinary-team huddle and other required meetings.
  • Assume the role of primary nurse, floor nurse, or triage nurse as directed by the Clinical Nurse Manager or Site Director.
  • Perform duties in accordance with Fallon Community Health Plan and Summit ElderCare policies and procedures.
  • Independently administer medications and treatments ordered by the provider.
  • Independently perform the home care nurse role, nursing assessments, and execute the plan of care in all settings where participants may be located.
  • Perform medication administration, bladder scans, electrocardiograms, point-of-care testing, and glucometer testing.

  • Ambulatory care or home care experience is helpful.
