Requirements
  • Current state Licensed Vocational Nursing (LVN) license.
  • Current CPR certification.
  • Reliable transportation to the office and patients’ homes.
  • Ability to perform the physical demands of the position, including bending, reaching, standing, stooping, sitting, twisting, talking, hearing, carrying equipment and supplies, and lifting, pushing, pulling, and supporting patients or equipment weighing 50 pounds or more.
Responsibilities
  • Provide direct patient care to infants, children, and adolescents primarily in their homes according to the active plan of care provided by the physician and other appropriate personnel.
  • Provide services in accordance with physician orders and applicable state, federal, and national accreditation commission regulations under the direction and supervision of clinical supervision staff and the agency administrator.
  • Perform comprehensive patient assessments.
  • Administer prescribed medications, treatments, and therapies as ordered.
  • Document timely, clear, concise, and accurate clinical notes in accordance with the plan of care.
  • Notify the registered nurse case manager of changes in the patient’s condition.
  • Provide and document education to patients and their families regarding diagnosis, treatment, and prevention.
  • Collaborate with other professional disciplines to ensure patients receive quality care needed to achieve and exceed individual goals.
  • Use appropriate infection-control procedures and safety practices.
  • Comply with company policies, practices, and procedures.
  • Perform other duties as assigned.

  • One year of home health experience.
  • One year of pediatric experience.

Provides in-home healthcare for children up to 18 with special needs, offering nursing, speech, physical, and occupational therapy, plus case management. A team of licensed clinicians visits the child’s home to deliver individualized therapies and nursing care, while case managers coordinate services across disciplines for comprehensive, continuous care. The focus on pediatric home health with a multidisciplinary, in-home approach and ethical, quality care sets it apart from broader healthcare providers. The goal is to improve health outcomes and daily function for children with special needs while supporting families through high-quality, home-based care.
