Requirements
  • Graduate of an accredited nursing program
  • Licensed Vocational Nurse (LVN) license required
  • Current LVN license in the State of California
  • CPR certification
Responsibilities
  • Assess, identify, manage, and treat skin conditions including abrasions, ulcers, infections, and other dermatologic concerns
  • Provide wound care treatments and monitor healing progress in accordance with physician orders
  • Perform comprehensive skin assessments and quarterly dermatologic evaluations
  • Provide therapeutic nursing care under physician or Medical Director supervision
  • Ensure residents with pressure injuries receive appropriate prevention and treatment interventions, including turning schedules, skin monitoring, and nutritional support
  • Provide direct nursing care in emergency situations or when illness or disability requires intervention
  • Assist with resident admissions, transfers, and discharges as needed
  • Support rehabilitation activities including range of motion exercises, ambulation assistance, and emotional support when appropriate
  • Collaborate with the Interdisciplinary Care Plan Team to develop and implement comprehensive care plans
  • Provide written and oral reports to the attending physician, Medical Director, and Director of Nursing regarding resident status and treatment outcomes
  • Confer with physicians and nursing leadership regarding dermatologic care needs
  • Participate in care planning meetings and facility committees as assigned
  • Assist with coordinating services with outside agencies and family members when necessary
  • Accurately document resident assessments, treatments, and responses in the medical record
  • Maintain confidentiality of resident information and Protected Health Information
  • Report suspected or known violations of privacy or unauthorized access to facility systems
  • Maintain records of resident complaints and grievances and report concerns to the Director of Nursing
  • Ensure resident rights are honored, including respecting refusals of treatment in accordance with facility policies
  • Follow established infection prevention and safety protocols
  • Report occupational exposures to blood, body fluids, infectious materials, or hazardous chemicals in accordance with facility policy
  • Use appropriate personal protective equipment including gloves, gowns, masks, and safety equipment when necessary
  • Assist in monitoring and maintaining adequate supplies of wound care products and medical supplies
  • Participate in in-service training programs related to wound care and dermatologic nursing practices
  • Assist with staff education related to skin integrity and treatment protocols as requested by nursing leadership
  • Attend continuing education programs to maintain professional licensure and remain current in clinical practice
  • This position may assist with guidance and support of nursing staff regarding wound care protocols and skin integrity monitoring.
