Requirements
  • Registered nurse license, Bachelor of Science in Nursing
  • Good standing as a licensed Registered Nurse with the State of Texas Board of Nursing; current BLS certifications (PALS preferred), First aid, CPR, Emergency behavior intervention
  • At least 5 years of experience as a RN
  • Pass a pre-employment drug screen and random drug screens throughout employment
  • Provide proof of work eligibility status upon request
  • Pass a pre-employment and biennial criminal background checks
  • Demonstrate the ability to work collaboratively with other staff members, service providers and professionals
  • Demonstrate the ability to communicate effectively in writing and verbally in English
  • Work in a fast-paced environment and make decisions based on the needs of the program and service population, while maintaining emotional control and professional composure at all times
  • Maintain computer literacy required to meet the responsibilities of the position
  • Work effectively and without intensive supervision both independently and as a member of a multidisciplinary team
  • Demonstrate a working knowledge of all Compass Connections policies and procedures
  • Must provide current physical-medical release and provide proof of immunizations, titers, or declinations for MMR, Varicella, Hep B, Hep A, Tetanus, PPD, Meningococcal (preferred)
  • Must be able to perform physical activity such as extensive walking, stretching, and bending
  • Must be able to work in a team environment
  • Must be able to maintain professional people skills, including oral, written, listening, and non-verbal communication skills
  • Compass Connections immunization policy requires annual influenza vaccination or approved exemption; this is a condition of employment

Responsibilities
  • Meet and maintain all federal and state regulatory guidelines and standards applicable to this position.
  • Provide telehealth triage services to assess patient needs and evaluate symptoms; this includes answering patient inquiries, providing patient teaching, and determining the urgency and appropriate course of action.
  • Conduct in-person and virtual assessments based on established protocols and accurately document medical findings.
  • Gather, collect, and clarify patients’ current condition and past medical history.
  • Effectively communicate with Medical Director as needed for reporting findings which are concerning for patient’s health and well-being.
  • Communicate with RN Managers and other medical staff to devise individual plans of care and acts as a nurse support to other medical staff.
  • Implement Compass Connections safety protocols including evacuating with children and other staff in case of an emergency.
  • Maintain confidentiality of health care records and information in keeping with HIPAA and Compass Connections confidentiality and Personally Identifiable Information (PII) protocol and procedures.
  • Address other job duties as assigned or for unforeseen matters that require attention to maintain a fluid and fully functional program.
  • Plan, organize and, prioritize daily work using sound judgement.
  • Monitor established health care plans for service population at the sites within the timeframe established by our State and Federal partners.
  • Ensure accurate documentation in accordance with agency policies and procedures, regulatory requirements and standards, and program timeline requirements.
  • Meet all program and federal partners’ documentation and processes requirements.
  • Other duties as assigned.
