Requirements

Responsibilities
  • Provide direct clinical services to Primary Care patients including brief assessment and evidence-based psychotherapy interventions for patients with medical, mental health, and substance use disorders.
  • Perform advanced, comprehensive assessment and analysis of complex psychosocial factors and coordination for patients receiving medication supported recovery.
  • Advise Primary Care providers and medical team members on the full spectrum of behavioral health and substance use conditions/diagnoses and appropriate/available behavioral health and substance use treatment options.
  • Empower patients' holistic healing by guiding primary care patients through evidence-based psychotherapy to foster resilience and recovery from medical conditions through a biopsychosocial lens.
  • Tailor care plans to each patient's unique needs and preferences, collaborating closely with primary care teams to deliver comprehensive, patient-centered support.
  • Leverage understanding of the interplay between physical and mental health to optimize treatment plans and patient outcomes.
  • Embrace a dynamic hybrid model of care blending in-person visits with telehealth/telemedicine to provide accessible support.
  • Provide mental health and substance use support to patients of all ages using evidence-based psychotherapy to empower recovery.
  • Create a trauma-informed care environment, guiding patients through personalized care plans.
  • Respond to crises with immediate assessment and intervention for individuals facing suicidal thoughts or other safety concerns.
  • Tackle intricate cases with advanced behavioral health assessments, coordinating multi-faceted care plans, including those receiving medication-supported treatment.
  • Empower primary care teams with behavioral health consultation, bridging physical and mental well-being for holistic outcomes.
  • Foster a network of support around each patient, collaborating with providers, staff, family members, and community resources.
  • Share insights on psychosocial treatments, behavioral barriers, and evidence-based practices with primary care teams, while upholding culturally sensitive care and community engagement.

  • Certified Alcohol and Drug Counselor (CADC)
