Discharge Planning Assistant - Social Work - Westchester Hospital–DayShift
AcrossNewYork-Presbyterian/Westchester Hospital the interdisciplinary team approach to care coordination drives our quality outcomes and amazing patient stories. As a Discharge Planning Assistant,you’llassistwithfacilitatingsuccessful discharge plans which are critical to length of stay reduction and patient flow.
Join a team of Care Management, Social Work, and Nursing, in providinga high levelof customer service to patients by ensuring a proper andtimelydischarge. Make regular rounds of all patients toassurethat the thoughtfully crafted treatment plans are being implemented and communicated across the interdisciplinary team. Goabove and beyond by securing resources for patients, contacting providers and community referrals to obtain authorizations, and setting up placement after discharge.
This isafull timedayshiftopportunity, covering the hours of9:00am –5:00pm,Tuesday – Saturdayat NewYork-Presbyterian/Westchester Hospital in Bronxville; right across from the Metro North Station.
High school diploma or GED
1 year of experience in a healthcare setting
