Requirements
  • A master's degree in Speech Language Pathology, Communication Disorders, or a related field, or equivalent education and experience, is required.
  • The necessary education to attain a Speech Therapy license in California must be completed.
  • A California Speech Language Pathologist license is required upon hire.
  • National Provider Basic Life Support certification from the American Heart Association is required within 30 days of hire.
  • The role requires the ability to stand and walk regularly, use manual dexterity, lift and move objects up to 50 pounds, and use standard medical equipment safely.
  • The role requires adherence to infection control guidelines, safety standards, fire and disaster plans, and mandated in-service training.
Responsibilities
  • Assess SLP care needs following referral by obtaining an accurate history and physical examination.
  • Document assessment data within 24 hours of each home visit.
  • Interpret assessment data holistically, including psychological, physiological, cognitive, and medication-related factors.
  • Determine reimbursement eligibility and communicate with insurance companies or health maintenance organizations regarding home-care eligibility.
  • Prioritize client care based on client needs and available resources, and manage time effectively.
  • Develop, coordinate, revise, and update individualized written plans of care, including specific speech-language pathology interventions, referrals, measurable goals, and outcomes.
  • Collaborate with clients and significant others to develop realistic plans of care.
  • Provide treatment interventions safely and competently according to the care plan, established priorities, approved procedures, protocols, and standards of care.
  • Plan and complete home visits in a timely manner and notify the supervisor of deviations outside the norm.
  • Apply technical speech-language pathology skills appropriately and carry out physician orders as directed.
  • Complete documentation in the required format and meet Medicare, Medicaid, and other applicable requirements.
  • Provide clients and significant others with information about diagnoses, interventions, and available resources.
  • Advocate for clients by promoting privacy, self-determination, confidentiality, self-care, and independence.
  • Investigate and follow up to ensure client needs are met, including providing telephone consultation about rehabilitation care.
  • Complete agency documentation, including daily schedules, timesheets, mileage reports, unusual-occurrence reports, employee-injury reports, and paid-time-off requests.
  • Complete discharge summaries.
  • Assess, plan, implement, and evaluate the educational needs and readiness of clients and significant others; develop and document individualized teaching plans and provide practical teaching materials and demonstrations.
  • Assist patients and caregivers in obtaining equipment and supplies needed for quality care.
  • Assess risk behaviors, develop risk-reduction plans such as home safety inspections, and integrate preventive care and health promotion during home visits.
  • Identify potential hazards, participate in resolution, and comply with safety and infection-control requirements.
  • After 72 hours, return materials to the manager for final review.
  • Arrange and complete supervisory visits for performance evaluations and submit supervisory visit forms to the manager or supervisor within 48 hours.
  • Complete new staff orientation, paperwork, and skills assessments; help design plans to refine skills when deficiencies are identified.
  • Review program changes, new employee paperwork, and results with the manager or supervisor.
  • Act as the point person for discipline-specific issues in the manager's or supervisor's absence, including staffing, clinical questions, and orientation of registry staff.
  • Participate in the Utilization Review committee and complete assigned work.
  • Assist the manager with interviews by assessing the clinical skills and competency of speech-language pathology, occupational therapy, and physical therapy candidates.
Desired Qualifications
  • One year of clinical experience as a Speech Pathologist is preferred.
  • Home health experience as a Speech Pathologist is preferred.

Compassus appoints Chris Core as chief growth officer to lead home-based care expansion
