Requirements
  • Experience in care management or social work with Medicaid/Health Related Social Needs (HRSN)
  • Strong communication and organizational skills

  • Ability to travel up to 25% for on-site service, and operate in multiple environments including high-volume social service settings

Responsibilities
  • Provide oversight and direction to the Social Care Network Care Management team
  • Coordinate with Divisional Headquarters team to ensure collaboration between Care Managers and non-local Salvation Army Centers
  • Monitor data entry and compliance of the Care Management team
  • Provide case/care management to Medicaid Members including screening, assessment, social care plan, and direct service delivery for health-related social needs
  • Manage incoming referrals for enhanced HRSN services and ensure timely connections for community members
  • Assess clients for Level 1 and Level 2 services and conduct outreach per required frequency, modality, and timeframe
  • Maintain caseload levels to sustain position through SCN billable activities within 3 months
  • Conduct HRSN screening using the Accountable Health Communities screening tool and manage Member consent and attestation
  • Perform eligibility assessments and refer Members to eligible programs and services (Level 1 and Level 2)
  • Develop and update social care plans detailing Member needs, eligibility and referred services
  • Document progress notes and actions with referrals and services; ensure data entry in SCN client database
  • Operate out of primary location and provide Care Management support to Salvation Army centers across SCN footprint; travel as needed
  • Engage Corps leadership and staff as needed
  • Communicate regularly with Divisional Headquarters Social Services team to discuss progress and challenges
  • Complete all required training and participate in annual professional development
  • Represent The Salvation Army at SCN and community meetings; attend staff meetings
  • Adhere to Salvation Army Policies and Procedures
Desired Qualifications
  • Leadership experience in a health care or social services setting
  • Experience with Medicaid, HRSN, or social care networks
  • Experience with data management and reporting for social programs
  • Bachelor’s or Master’s degree in Social Work, Public Health, Health Administration, or related field
  • Proficiency with Microsoft Office Suite and database systems
  • Knowledge of community resources and ability to build partnerships
