Requirements
  • Maintain an active, unencumbered license to practice nursing.
  • Hold ANCC or AANP board certification as a Nurse Practitioner in Family, Adult, Gerontology, or Emergency Medicine.
  • Maintain current CPR certification.
  • Complete recommended healthcare personnel vaccination and preventive testing requirements before hire, including Hepatitis B, influenza, MMR, Tdap, varicella, and tuberculosis testing.
  • Be able to communicate effectively with elderly and chronically ill patients and their families.
  • Practice autonomously in a remote clinical environment, including independently conducting patient assessments, formulating evidence-based treatment plans, managing complex chronic conditions, and making clinical decisions without direct on-site supervision.
  • Be proficient with computer platforms, electronic health records, secure messaging systems, and cloud-based documentation tools.
  • Be available for a minimum of 25 appointment slots per month, approximately 37 hours.
  • Comply with company policies, procedures, confidentiality requirements, HIPAA requirements, and applicable data-handling and service standards.
Responsibilities
  • Complete comprehensive, accurate, and thorough reviews of assigned members, including timely completion and submission of required encounter documentation.
  • Document all pertinent and active medical conditions in the medical record in compliance with CMS/DHHS, company policy, and client requirements.
  • Support the physician-patient relationship through timely communication and documentation of assessment findings, recommendations, additional service needs, emergency services when necessary, and follow-up requirements with the primary care provider and health plan.
  • Provide members with general recommendations to improve their understanding of chronic conditions, including information about recommended testing.
  • Address and close identified disease-specific and preventive gaps in care.
  • Recognize emergent or urgent situations requiring escalation and take appropriate action according to company policies, professional judgment, and ethical practice standards.
  • Perform, document, and communicate results of point-of-care testing.
  • Maintain compliance with company policies, procedures, and mission statement.
  • Fulfill duties reasonably assigned to support the company's operational and financial success.
  • Uphold separation-of-duties responsibilities for applicable processes and procedures.
  • Perform other duties as assigned.
Desired Qualifications
  • Home health experience.
  • Understanding of Medicare, Medicaid, and health plan benefit structures.
  • Bilingual or multilingual ability.
  • Knowledge of CMS regulations and NCQA HEDIS guidelines.
  • Knowledge of evidence-based clinical practice standards from the American Diabetes Association, American College of Cardiology, and American Heart Association.
  • Familiarity with ICD-10 and CPT-4 coding practices.
  • Ability to multitask.
  • Customer service skills.
