Requirements
  • Bachelor’s degree in human service related field and two years’ paid human service experience or equivalent combination of education and experience.
  • Must be a QHP as defined by OASAS and/or OMH Operating Regulations.
  • NYS motor vehicle license, safe driving record and availability of personal vehicle for work.
  • Demonstrated ability to provide comprehensive assessment and evaluations of persons with diverse psychiatric and/or chemical dependency diagnoses who are referred for residential recovery services.
  • Basic Computer Literacy as evidenced by the ability to go online and conduct basic searches on the Internet, check his/her email account, create and send email with an attachment and use simple email features.
  • Basic Microsoft Word skills as evidenced by the ability to produce basic business letters and simple tables and charts; as well as check spelling, set tabs, change page orientation, sort, save, open and organize electronic files.
  • Experience using multiple database programs required.
Responsibilities
  • Evaluation and coordination of admission of referrals.
  • Serves as liaison to referral sources and responds to general inquiries regarding referrals.
  • Screen and assess referrals, evaluating individuals for admission suitability.
  • Assesses and evaluates individuals who are referred to determine their appropriateness for admission.
  • Coordinates Admission Safety Reviews of clients who meet risk criteria.
  • Prepares summaries of client evaluations.
  • Communicates outcome of admission decision to referral sources and referred individuals.
  • Coordinates placement in appropriate unit for individuals who have been accepted for admission.
  • Provides outreach and information to potential referral sources about East House services.
  • Coordinates flow of referrals and evaluations to maximize the utilization of beds.
  • Report on referral status, trends, and service utilization to agency staff and community service providers.
  • Maintains documentation to the standards defined by regulators, funders and East House policies and procedures.
  • Successful recertification reviews indicating that admission criteria have been met.
  • Documentation shows that admission criteria are consistently met.
  • Appropriately identifies referrals that meet criteria for Admission Safety Review.
  • Community referral sources, clients and East House programs are satisfied with admission processes.
  • Community is educated about East House programs and referral processes.
  • Programs meet occupancy goals.
  • Meets documentation standards as evidenced by audits, chart reviews, and recertification review.

East House provides housing and recovery support services for individuals recovering from mental health and substance use disorders. It serves over 1,200 people each year, primarily in Monroe, Genesee, Livingston, Orleans, Wayne, and Wyoming Counties.
