Requirements
  • A master's degree from an accredited school of social work is required, or an equivalent combination of education and experience.
  • State licensure as a Licensed Medical Social Worker is required within one and a half years, or a limited permit not exceeding one year or New York State licensure upon hire if duties are performed under the Office of Mental Health.
Responsibilities
  • Provide social work services to a patient care unit when the assigned social worker is unavailable for a significant period of time, including coverage during extended absences or vacancies.
  • Coordinate discharge planning for patients determined to be at psychosocial risk in collaboration with health care team members, and provide appropriate handoffs when patients transfer to other hospital units.
  • Serve as a resource consultant to patients and families, facilitate referrals to appropriate community agencies and hospital resources, and conduct home visits when appropriate for psychosocial assessments and social work interventions.
  • Provide social work interventions, including risk screening through chart review and interdisciplinary rounds, psychosocial assessments for identified risk factors, crisis intervention, supportive counseling, and grief and bereavement counseling.
  • Handle risk management issues in close consultation with the supervisor and Clinical Manager.
  • Document social work activity in the electronic medical record.
  • Provide social work coverage to other clinical areas or programs during coworkers' absences as requested.
  • Complete hospital- and Social Work Division-required in-service training and ongoing professional development activities, including training designed to enhance knowledge and skills in protective work.
  • Complete required Employee Health appointments within the required timeframe.
  • Attend appropriate Social Work Division meetings, including division meetings, complex case discussions, and transplant staff meetings.
  • Supervise social work students pursuing a Master of Social Work or Bachelor of Social Work degree.
  • Orient and provide ongoing peer support to new social work staff.
  • Perform other duties as assigned.
