Requirements
  • A Bachelor's degree in Social Work is required.
  • At least one year of post-bachelor's social work experience in a health care setting is required.
  • The candidate must be able to reliably handle and maintain confidential information.
  • The candidate must have a valid driver's license and an insured automobile in good working order.
  • The candidate must be able to communicate effectively verbally and in writing.
Responsibilities
  • Conduct a psychosocial assessment within five days of hospice admission, including emotional, social, financial, and environmental resources, and identify appropriate psychosocial problems, interventions, and goals for the patient's plan of care.
  • Participate in interdisciplinary group meetings and develop the patient's plan of care.
  • Observe, record, and report changes in the patient's emotional and social factors, including the patient's response to illness, care needs, and treatment.
  • Maintain and submit required written clinical records, including the initial evaluation, care plan, and daily notes.
  • Evaluate patients' and families' responses to and the effectiveness of medical social-work interventions.
  • Confirm weekly visit scheduling with patients' case managers to coordinate visits by other staff.
  • Participate in interdisciplinary group and Quality Assurance and Performance Improvement meetings and activities.
  • Continue offering support to the interdisciplinary group and monitoring the evolving needs of patients and caregivers when social-work services are declined.
  • Participate in discharge planning as needed.
  • Pursue ongoing professional development through formal education, workshops, conferences, professional meetings, research, and reading.
  • Assess patients' and family members' ability to cope with the patient's dying.
  • Consult with health-team members to help them understand social, emotional, and environmental factors related to patients' health problems.
  • Help patients use family and community resources.
