Requirements

Responsibilities
  • Performs comprehensive nursing assessment and reassessment, including age-appropriate admission or transfer assessment and documenting findings; assesses post-operative/post-invasive procedure patients; evaluates education and discharge needs of patient and family on admission and throughout hospitalization; establishes, coordinates and evaluates a plan of care based on assessment data, patient diagnosis, lab data, tests, physician orders, protocols and standards of care; identifies short and long term goals and formulates nursing interventions to achieve desired outcomes; incorporates disease specific evidence based practice into care plan and documentation; provides and documents nursing interventions based on assessed patient needs and changes in patient status; collaborates with health team members for coordination of daily plan of care, communicates handoff reports, administers and documents medications accurately, monitors IV fluids and parenteral nutrition per policy; completes referrals, requests consultations for special needs or information for patient and family; provides patient/family education and discharge planning; clarifies and implements physician orders; assists physicians with procedures or delegates to Care Partner as appropriate; documents readback of telephone/verbal orders and takes such orders only in emergencies; recognizes changes in patient condition and takes appropriate nursing actions; involves family/guardian in care and decision making as appropriate; recognizes risks and applies infection control practices; expands specialized care to high risk patients including restraint care, pain management and abuse assessment; demonstrates accountability for professional practice including adherence to policies and participation in meetings and governance; maintains currency of hospital/unit information, policies and procedures; demonstrates ability to adapt to changing work demands; relates to coworkers professionally and maintains collaborative team approach; orients and precepts new employees; serves as a resource to patients, families, medical staff and health care team; conducts effective handoffs; communicates plan of care clearly; demonstrates patient and family education; follows safety and infection control policies; participates in mandatory annual requirements and performance improvement activities; provides age-appropriate care; utilizes resources efficiently and contributes to cost-effective services; adheres to hospital standards for service, communication, and confidentiality; participates in performance improvement, nursing research and quality indicators; and performs other related duties as assigned.
