Requirements
  • At least 2 years of experience working with frail or elderly disabled adults and caregivers.
  • Experience with data input, case review, and case note entry.
  • Working knowledge of activities of daily living and instrumental activities of daily living assessments.
  • A valid driver's license and an insured vehicle for travel within Georgia.
  • Verbal and written communication, organizational, and time management skills.
  • Critical thinking and problem-solving skills.
  • Proficiency in Microsoft Word, Excel, and Outlook.
  • Training skills.
Responsibilities
  • Conduct home orientations involving the signing of paperwork.
  • Perform initial assessments during orientations.
  • Conduct periodic supervisory visits and check-ins for active cases.
  • Train caregivers and patients as required for each case.
  • Interact with caregivers and other stakeholders as needed.
  • Participate in interdisciplinary meetings with care managers, caregivers, and patients.
  • Collaborate in care practice and communicate case impressions to appropriate staff.
  • Critically assess procedures and implement improvements for case evaluation, feedback, and caregiver training.
  • Travel within Georgia, including the Atlanta metropolitan area.
Desired Qualifications
  • Previous field experience in a medical or case management environment.
  • Hands-on experience in home care.
  • Salesforce experience.
