- Graduate of an accredited school of vocational/practical nursing.
- Hold an active state license as a licensed vocational/practical nurse or a multistate license issued by a Nurse Licensure Compact jurisdiction.
- Have one year of experience as a licensed vocational/practical nurse in a clinical care setting.
- Demonstrate nursing skills consistent with generally accepted standards of practice.
- Provide proof of current cardiopulmonary resuscitation certification.
- Have reliable transportation and valid automobile liability insurance.
- Under the direction of the registered nurse, assist in identifying the patient's physical, psychosocial, and environmental needs through documentation.
- Be present within partnering communities and clients' independent residences.
- Communicate effectively with patients, families, team members, and other healthcare professionals through clinical notes, case conferences, and communication notes.
- Complete appropriate documentation in a timely manner.
- Monitor assigned cases to ensure compliance with third-party payor requirements.
- Participate in agency-sponsored in-service training.
- Promote the agency philosophy and administrative policies to ensure quality of care.
- Participate in planning and implementing care with the patient, representative, caregiver(s), and registered nurse.
- Provide physician-ordered services indicated in the plan of care.
- Provide care using infection-control measures that protect staff and patients.
- Provide patient and caregiver education.
- Participate in the agency's Quality Assurance and Performance Improvement program.
- Maintain professional growth and competency.
- Assure continuity of care through delivery of quality patient care.
- Home health experience is preferred.
- Experience with the Homecare Homebase platform is a plus.
- Good interpersonal skills.
