Requirements
  • A bachelor's degree in a Human Services field plus at least one year of experience working in a Human Services field, or an associate's degree in a Human Services field plus at least three years of related experience working in a Human Services field, or a high school diploma/GED plus at least five years of experience working in a Human Services field.
  • A clean and valid New York State driving record as outlined in DePaul's personnel policy.
  • Must be at least 18 years of age.
Responsibilities
  • Work with management staff to provide prospective residents with tours and lunch visits, then provide input to the Admission/Management team regarding the individual's acceptance into the program.
  • Ensure each resident receives comprehensive, person-centered service planning throughout the resident's tenure, including recovery-oriented services and ongoing discharge planning, in accordance with standards of practice and the New York State Office of Mental Health.
  • Conduct and complete quarterly service plan reviews with residents and their support teams to assess progress and revise or update plans when necessary.
  • Provide teaching, monitoring, and recovery-oriented support in rehabilitative services including assertiveness and self-advocacy training, community integration and resource development, daily living skills training, health services education, medication management and training, parenting training, rehabilitative counseling, skill development, socialization, substance abuse services, and symptom management.
  • Observe, monitor, document, and report resident behaviors and progress in accordance with agency standards.
  • Maintain accurate and timely resident files, including progress notes, service plan updates, psychiatric and medical records, and other pertinent resident information.
  • Ensure annual physical, eye, medical specialist, and dental appointments are scheduled and arrange transportation when necessary.
  • Collaborate with the resident's Health Home Care Manager and other community service providers to ensure continuity of care.
  • Serve as a liaison between residents and their clinical providers regarding monitoring of mental health and well-being.
  • Provide direct assistance with daily living skills, including room upkeep, simple meal preparation, and laundry when indicated.
  • Help residents plan purchases of clothing and other necessities.
  • Work with residents to encourage healthy connections with family and communities and develop new supports and interests.
  • Coordinate and participate in recreational activities when indicated.
  • Assist Medication Coordinators with supervision of resident medications when necessary and assume dining room responsibilities during breakfast when assigned.
  • Safely transport residents when necessary.
  • Maintain resident confidentiality and demonstrate a caring, positive attitude toward residents, staff, guests, family members, and service providers.
  • Follow all safety rules and regulations for self, residents, and staff.
  • Attend staff meetings and training as required by the supervisor.
  • Work as a member of the residential team, provide relevant input for treatment/support team meetings, and participate in continued stay reviews.
  • Perform other duties necessary for effective program operation and resident well-being.

Pan-American Square Apartments wins Community Impact Award. SWBR Company is pleased to announce that DePaul Pan-American Square Apartments has been recognized with the 2025 Community Impact Award by the Buffalo/WNY Chapter of the American Institute of Architects. This award honors projects that make a meaningful difference in their communities through thoughtful, inclusive design. Completed earlier this year, Pan-American Square Apartments transformed a former rail corridor into a 150-unit affordable and supportive housing community in Buffalo, NY. Designed to Passive House standards, the development provides energy-efficient homes and essential services for seniors and individuals experiencing homelessness or mental illness. More than housing, it is a catalyst for community revitalization, delivering stability, sustainability, and hope for Buffalo's most vulnerable residents. Jury Comment: The integration of community-based programs and sustainable design at the Pan-American Square Apartments struck the jury. Exemplifying the criteria of the Community Award, the Pan-American Square Apartments took a holistic approach to senior living and supportive housing that will have a lasting positive impact on the WNY community. It is clear that this project will uplift and provide stability for the most vulnerable members of its community and the design team took every opportunity to maximize resources available to its residents. SWBR provided architectural, interior design, structural engineering, landscape architecture, and graphic design services for the project. The awards celebration was held Thursday, November 6 in Buffalo, NY.
