Requirements
  • Graduate of an accredited practical nurse or vocational nursing program
  • Minimum of one (1) year of experience (per state regulations) preferably in the area of home care or med/surge and shall work under supervision of an RN
  • Current licensure in state and CPR/BLS certificate
  • Complies with accepted professional standards and practice
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist
  • Prolonged or considerable walking or standing. Able to life, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills
  • Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order
Responsibilities
  • Implements plan of care initiated by the clinician
  • Provides accurate and timely documentation consistent with the plan of care
  • Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care
  • Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietitian) to the clinician and/or clinical manager
