Requirements
  • Current Registered Nurse licensure in good standing in the state or states of practice, with one year of experience under the license.
  • Ability to collaborate within a team environment with nurses, social workers, chaplains, volunteers, and certified nursing assistant leadership.
  • Ability to work independently with patients, phone, email and written communication, meetings with healthcare professionals, and hospice rules and regulations.
  • Time-management skills to manage visits, documentation, meetings, scheduled visits, required meetings, urgent needs, and multiple competing concerns.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, a valid driver's license, and auto insurance to travel across the service area.
  • Strong clinical and communication skills.
  • Passion for hospice care and ability to advocate for hospice services and Suncrest Hospice.
Responsibilities
  • Provide care and services for a group of hospice patients in private homes, assisted living facilities, and hospitals.
  • Complete hospice admissions and patient visits as needed.
  • Answer calls from patients, families, and customers.
  • Make nursing visits for symptom management, deaths, and admissions as needed.
  • Work after hours on-call for seven days or nights on, followed by seven days or nights off.

  • Admissions and on-call experience is preferred.
