Requirements
  • An active Registered Nurse license in the state of employment is required.
  • Graduation from an accredited school of Nursing is required.
  • Flexibility and confidence working across varied patient assignments are required.
  • Strong assessment skills and sound clinical judgment under pressure are required.
  • Excellent documentation habits and communication skills are required.
  • A valid driver's license and willingness to travel locally are required.
Responsibilities
  • Deliver professional nursing care across the full nursing process within the scope of practice.
  • Conduct thorough patient and family assessments and contribute to individualized care plans.
  • Complete all visit types as needed to meet patient and family needs.
  • Administer medications and treatments according to physician orders.
  • Identify nursing diagnoses from health assessment data and respond accordingly.
  • Record accurate, timely clinical notes, including pain and symptom management outcomes.
  • Communicate changes in patient condition to physicians and obtain orders as required.
  • Coordinate with community health partners to support seamless care transitions.
  • Ensure equipment and necessary services are arranged and available for patients.
  • Maintain and grow clinical skills through continuing education and in-service participation.

  • Clinical experience in hospice, home health, or a related setting is preferred.
