Requirements
  • A bachelor's degree in Nursing and five years of inpatient clinical experience with knowledge of complex disease processes is required, or a bachelor's, master's, or doctorate degree in a relevant clinical discipline with five years of inpatient clinical experience and knowledge of complex disease processes.
  • An equivalent combination of education and experience may be accepted.
  • A current New York State Registered Nurse, Nurse Practitioner, Physician Assistant, Doctor of Medicine, or Doctor of Osteopathic Medicine license is required upon hire.
Responsibilities
  • Facilitate clinical documentation improvement through concurrent and retrospective interaction with physicians and members of the healthcare team.
  • Recognize gaps and facilitate modifications in clinical documentation that support the accuracy of medical conditions for inpatient encounters.
  • Recognize documentation opportunities with the assistance of computer software and conduct clinical documentation improvement efforts through query processes.
  • Provide oversight of EPARC, including utilization management and unbilled work queues.
  • Communicate with individual physicians and the medical team, providing guidance and clarification around principal diagnoses, complicating conditions, and diagnoses being treated to ensure complete and accurate documentation in the medical record.
  • Present overall findings to key stakeholders, including leadership and peers.
  • Provide education to peers and healthcare team members on documentation improvement, reimbursement opportunities, and overall performance.
  • Demonstrate knowledge of trends in clinical diagnoses that result in payer denials.
  • Ensure the validity of data for reporting and measuring physician and hospital outcomes through software and data analysis.
  • Audit clinical documentation to confirm points of clarification have been entered in the medical record.
  • Meet established productivity expectations of the clinical documentation improvement program.
  • Maintain knowledge of hospital clinical practice guidelines to support thorough review of the medical record.
  • Perform other duties as assigned.
Desired Qualifications
  • Prior experience with EPARC, eRecord, HDM, and SharePoint is preferred.
  • Clinical Documentation Specialist or Clinical Documentation Improvement Professional certification is preferred.
