Responsibilities
  • Oversees team member work in the discharge assessment and planning, from admission to discharge.
  • Assists Director in the implementation and coordination of the Hospital Length of Stay Committee.
  • Develops and maintains evidenced based care coordination standard work order sets and care paths and updates annually to reflect evidence-based care coordination practices.
  • Analyzes, interprets and makes recommendations based on care coordination outcome data.
  • Monitors adherence to standard work, tracks variances from protocols/care paths and reports data to team and organization.
  • Serves as a liaison to hospital clinical departments by providing feedback on care coordination outcomes and analyzing individual dept. performance with regard to established goals.
  • Ensures compliance with relevant CMS and commercial payor standards to maintain specialty contract compliance and completion of all intra-cycle reports to be submitted to payors within required timeframe.
  • Promotes a positive environment to patients, families, staff and the community.
  • Responds to inquiries and complaints effectively and timely and promotes a positive, professional image serving as a liaison between staff, physicians, and administration.
  • Demonstrates commitment to teamwork through positive interactions and feedback to/from subordinates, physicians, peers and other customers.
  • Accepts responsibility for employee morale and team performance toward goal achievement. Develops measurable, team goals, monitors progress and keeps staff abreast of progress toward achievement.

  • Continually assesses current trends/clinical/technological advances in the area of best practice and takes appropriate action to improve outcomes, patient satisfaction and cost effective care.
  • Reviews and updates department/program specific standards/policies/procedures as needed to assure compliance with established standards, standardization efforts and current practice.
  • Oversees the daily operation of the program and, in collaboration with physicians and senior leadership, appropriately adapts patient care processes and transitions of care to the assessed needs of the patient/family.
  • Assesses, provides for the delivery of and evaluates clinical practices and insures quality of services delivered within the program.
  • Insures that all staff collaborates in the discharge planning process and patient/family education to enable the system to deliver services more cost effectively in the most appropriate setting to meet patient needs.
  • Serves as an expert for complex and/or highly specialized patient care situations to hospital staff/personnel and other health care professionals within the organization and community.
  • Demonstrates, via role modeling and consultation, advanced knowledge and practice in the assessment, diagnosis, treatment and evaluation of human responses to actual and/or potential health problems.
  • Consults with healthcare providers, ancillary services, patients and significant others to assess and identify individual patient care needs.
  • Participates with System Care Coordination in the development and maintenance of standardized care coordination education materials.
  • Serves as a practice resource for care coordination team members.
  • Site specific expert resource for clinical excellence in continuum of care.
  • Sustain professional competency, skill development and specialty certification.
  • Identify PI opportunities to achieve compliance with best practice by all caregivers.
  • Track, trend and analyze data. Define opportunities for improvement from data analysis.
  • Facilitate development of action plans to improve care coordination outcomes.
  • Assists with the development and implementation of operational and capital budgets that are consistent with system-wide, facility/divisional goals and objectives.
  • Assists with the establishment of productivity measures and facilitates program and team achievement of established goals.
  • Maintains FTE, salary and expense budgets at or below budgeted levels, demonstrating the ability to review/analyze current/historical data relevant to budget variances.
  • Assists with the development of collaborative approaches that promote quality, cost-effective utilization services across the continuum.
  • Coordinates with System Care Coordination and Hospital Director on compliance and regulatory standards as well as contractual guidelines.
  • Assists in the development and implementation of professional standards for the program based upon recognized standards of care, JCAHO and CMS standards, state/federal regulations and overall system policies and procedures monitors staff compliance of regulatory standards.
  • Performs other duties as assigned.

Cobb's business leaders honored at 84th Annual Dinner. Highlights of the evening include the 2025 Chairman awards presentation and an address by the 2026 Chairwoman. The Cobb Chamber celebrated its many accomplishments of 2025 at the 84th Annual Dinner celebration, presented by Wellstar, on Feb. 28 at the Cobb Convention Center Atlanta. Over 1,200 business and community leaders attended the black-tie optional dinner affair, including military leaders, economic development partners, and elected officials. Brian Jordan, former NFL and MLB athlete and the President of the Brian Jordan Foundation, served as the event's master of ceremonies. The evening included a robust program, including outgoing Chairman, Chris Britton of Brasfield & Gorrie, passing the leadership to 2026 Chairwoman, Melissa Cantrell of CDH Partners. In his address, Britton celebrated the collective efforts of the Chamber and how it served as a champion for businesses and the community over the past year. He highlighted: * A record year for the Membership Campaign, raising a historic $1,230,533 and welcoming 336 new members. * 26 project wins for SelectCobb, bringing more than 2,181 high quality jobs. * MLB All-Star week, welcoming thousands of visitors to the county. During the evening, Britton presented several awards to honor individuals who have made significant contributions to enhance the quality of the Chamber's programming and the quality of life in Cobb. The 2026 Chairman's Award was presented to Al Martin of Georgia Power, a dedicated partner to the Chamber for several decades. The award is presented to an individual for outstanding leadership, dedication, enthusiasm, and commitment to the Cobb Chamber. Martin is a long-term investor in the Cobb Chamber, serving in the membership campaign as division chair, overall chair, team captain, and the top producer in 2025. He played a vital role in developing the mentoring component of CYP Cultivate, chaired the grants committee for Cobb Community Foundation, served as a long-time Cobb Travel & Tourism board member, and given his time to countless nonprofits like the Center for Family Resources. The 2026 Dr. Robert A. Lipson Award, was presented to Mike Plant, Atlanta Braves. The award is presented to someone who is a visionary, a friend, a mentor, and a leader. Since 2016, Fucito has served as the Chamber's Finance Chair, extending beyond his own retirement to ensure a smooth transition to his successor. Wellstar Children's Hospital of Georgia receives $50 million gift, to soon be renamed. AUGUSTA, Ga. (WJBF) - Wellstar Children's Hospital of Georgia on Monday announced it has received $50 million from Paychex founder Tom Golisano, the largest donation in the history of Wellstar and the Children's Hospital of Georgia. Accepting the gift means that the children's hospital will join the Golisano Children's Alliance, a national consortium of 12 leading children's hospitals, making it the first in all of Georgia, South Carolina, Alabama, Tennessee and North Carolina. The money will be used to created a new pediatric intensive care unit. "We've been on the journey and in the process of moving towards having a separate cardiac intensive care unit for children. This just accelerates that, and allows us to do it much quicker than we otherwise would have been able to do," said Dr. Valera Hudson, Pediatrician-In-Chief at Wellstar Children's Hospital of Georgia. "So, that's good news for children in Georgia and our region who need this care." The hospital will soon be renamed to Wellstar Golisano Children's Hospital of Georgia. "Children's hospitals are essential to the health and future of our communities, and each one serves families with unique needs and challenges," said Tom Golisano.
