Requirements
  • At least 2 years of experience working with frail or elderly disabled adults and caregivers.
  • Experience with data input, case review, and case note entry.
  • Working knowledge of activities of daily living and instrumental activities of daily living assessments.
  • A valid driver’s license and an insured vehicle for travel within Indiana.
  • Strong verbal and written communication, organizational, time management, critical thinking, and problem-solving skills.
  • Proficiency with Microsoft Word, Excel, and Outlook.
  • Excellent training skills.
Responsibilities
  • Evaluate clients’ home care needs and assess the care environment.
  • Assess the caregiver’s fitness to provide care.
  • Conduct home orientations, including signing paperwork.
  • Perform initial assessments during orientations.
  • Conduct periodic supervisory visits and check-ins for ongoing cases.
  • Train caregivers and patients as required for each case.
  • Interact with the patient’s care manager as needed.
  • Participate in interdisciplinary meetings with care managers, caregivers, and patients.
  • Collaborate on care and communicate case impressions to appropriate staff.
  • Critically assess procedures and implement improvements for case evaluation, provide feedback to appropriate parties, and train caregivers.
  • Travel throughout Indiana, with mileage and expense reimbursement.

  • Previous field experience in a medical or case management environment.
  • Hands-on experience in home care.
  • Salesforce experience.
