  • Doctor of Pharmacy from an accredited program
  • Licensed and in good standing with the pharmacy board of their state, and willingness to get licensed in multiple states
  • 3-5 years of applicable direct clinical practice experience in primary/ambulatory care
  • Comfortable discussing a wide range of medical conditions and pharmacy benefits
  • Highly empathetic. You'll work with patients and their families who are going through challenging times
  • Efficient. We are a fast growing company and we are busy! You will have lots of work to do and will be expected to meet volume goals without sacrificing quality
  • Flexible and comfortable with working in a rapidly-changing environment. Included Health is constantly evolving, and our clinicians need to be able to thrive in these types of situations
  • Strong verbal and written communication skills. You will spend a lot of time on the phone with members and families, therefore the ability to gather a clinical history, answer questions, and give a great phone experience is critical
  • Able to handle a fast-paced environment with competing priorities. You need good judgment to appropriately juggle your responsibilities
  • Able and willing to work until 6:00 pm local time, with occasional weekend commitments as well
  • Strictly follow security and HIPAA regulations to protect our members' medical information
  • Collaborate well across a multidisciplinary team with clinical and non-clinical members to deliver a seamless, top-quality care experience to members
  • Assess members' pharmacologic needs by performing Comprehensive Medication Reviews, ensuring they receive evidence-based, clinically appropriate care, and navigate members to providers or health care benefits to ensure high quality care
  • Serve as a clinician within state-specific scope of practice guidelines by performing medication therapy management and assisting with chronic disease management in collaboration with the primary care provider for members enrolled in Virtual Primary Care
  • Collaborate with an interdisciplinary team to create a care plan tailored to the member's needs
  • Serve as the subject matter expert on pharmacy benefits navigation and customer Pharmacy Benefit Managers for members enrolled in the Connected Care Program and Virtual Primary Care Program
  • Support the greater clinical team by consulting on pharmacy related questions
  • Evaluate response to medication therapy through either direct member involvement and clinical assessment or through collaboration with the primary care provider
  • Work with our training team to train clinical and non-clinical staff as appropriate on key pharmacologic clinical issues and navigation/coordination opportunities
  • Support in the development of Included Health's long-term clinical pharmacy strategy, as needed
Desired Qualifications
  • Completion of a PGY1 Pharmacy Residency program
  • Board Certification in Ambulatory Care or Pharmacotherapy (BCACP or BCPS)
  • Licensed and in good standing with the pharmacy board in multiple states
  • Clinical experience in primary/ambulatory care at an integrated health care system and/or health plan
