Requirements
  • A Bachelor’s Degree.
  • A minimum of 1–2 years of work experience participating in performance improvement programs, problem-solving, time management, or hospitality programs.
  • Excellent written and oral communication skills.
  • Strong prioritization skills and the ability to be self-motivated, detail oriented, and demonstrate an excellent work ethic.

Responsibilities
  • Implement Catholic Health’s Patient Experience Program on assigned hospital campuses.
  • Collaborate with clinical and nonclinical disciplines to create a safe and comfortable environment for patients and families.
  • Serve as a key point of contact between unit or department leadership and patients and families in managing and coordinating patient care.
  • Listen to patient concerns, escalate issues to appropriate management, and perform service recovery as needed.
  • Round on patients and families regularly to ensure standards of care are met and develop relationships of trust.
  • Facilitate communication among patients, families, and the care team.
  • Facilitate patient-experience rounding by senior leaders on the nursing unit and shadow and observe the rounding process.
  • Ensure real-time follow-up on patient and family concerns.
  • Identify and follow through with patients and families who have special needs or requests or require service recovery.
  • Track and trend patient feedback to identify issues and opportunities.
  • Complete reports and documentation for appropriate stakeholders.
  • Facilitate surveys, discuss them with patients and families, and encourage survey participation and unbiased feedback.
  • Conduct post-discharge follow-up phone calls to collect additional feedback.
  • Assist with capturing patient success stories.
  • Assist the Patient Experience Director with in-depth analysis of patient metrics, patient-experience surveys, written and verbal compliments and complaints, and qualitative data from multiple sources.
  • Provide one-on-one coaching for leaders and key staff to improve patient experience.
  • Communicate regularly with the campus Patient Experience Director about project timelines and progress.
  • Collaborate regularly with the campus Patient Experience Director.
  • Educate staff on patient-experience improvement philosophies and techniques using I-CARE values to drive culture and behavior.
  • Participate in unit and departmental meetings and huddles as indicated.
  • Assist in implementing process-improvement projects that affect patient experience in the hospital.
  • Perform other related duties as directed by the Catholic Health Office of Patient Experience.
  • Exemplify, promote, lead, and coach on I-CARE values and patient-experience behaviors with patients, families, and employees.
