We are filling a permanent and a limited duration Outreach Nurse position. The limited duration position is expected to last up to 6 months; however, it may be extended or end at any time depending on workload and program needs.
Summary of Duties
As an employee working within the Stabilization and Crisis Unit (SACU), you will be a part of a program that provides 24-hour residential care to Oregonians with intellectual/developmental disabilities, often with co-occurring mental health diagnoses, whose support needs exceed the supports offered or provided by other community-based residential programs. SACU provides development and support to assist individuals in safely transitioning back to their home community. We model positive attitudes while offering assistance with access to: health and medical supports, psychological services, emotional and behavioral support, educational supports. SACU staff also provide life skills training, recreational opportunities and community integration. SACU is dedicated to providing the support necessary to maintain the quality of life, achieve the highest possible level of independence and promote social opportunities, which benefit the individuals served and their communities.
We operate 18 residential group homes along the I-5 corridor from Portland to Eugene.
As an Outreach Nurse, you will:
Work in home health like atmosphere supporting individuals with I/DD in a group home setting providing support, training, and delegation of nursing tasks to non-licensed staff.
Work with a team of support staff, professionals, providers, guardians, and family members.
Create care plans and protocols to treat the needs of each individual in the assigned homes as well as train the direct support crisis specialist to implement the care plan and protocols.
Assist direct support crisis specialists to coordinate community medical appointments and attend when possible.
At times be required to delegate nursing tasks to ensure the health and safety of the individuals served. Due to the high behavior needs and diagnoses of the individuals served, you are required to have exceptional assessment skill and experience with mental health symptoms.
Minimum Qualifications
A valid driver’s license and acceptable driving record are required for this position.
Possess a current, full, active unrestricted Oregon Professional Registered Nurse license.
AND
Have experience in a home health care, clinic setting, skilled facility, and/or medical surgical unit that included full range triage nursing.

Working Conditions
You will work Monday through Friday 8:00 a.m. to 5:00 p.m. with flexible hours and the need to be on-call.
You may need to work evening and weekends.
There is frequent daily lifting of up to 75 pounds involving transferring, positioning, and moving individuals.
There is frequent daily squatting, bending, and stooping.
There is the potential for exposure to infectious diseases and body fluids.
You will work with individuals who experience an intellectual and/or developmental disability (I/DD) and co-occurring mental health disorders.
At times you will be exposed to individuals who are justice-involved or committed, and/or individuals who may often experience crisis that may be communicated in significant emotional or physical behaviors towards themselves or others.
You will have daily exposure to household strength cleaning agents, detergents.
You will be exposed to a variety of settings both within the residential group homes and the community.
You occasionally will be exposed to inclement weather (example: accompanying clients to doctor appointments).
You may be required to transport individuals using wheelchair accessible vehicles, passenger vans, mini vans and/or standard size cars. Must have the ability to drive a car/van.
You must maintain a valid State of Oregon Registered Nurse License.
You must maintain up-to-date CPR.
You will have frequent contact with the public and other agency personnel.
You will complete biannual Blood Borne Pathogen Training.
Background Checks and Requirements
If selected as a finalist, we will conduct a criminal history and background check. Adverse criminal history or background check may lead to disqualification.
The Oregon Department of Human Services does not offer visa sponsorship. Within three days of hire, you will be required to complete the US Department of Homeland Security’s I-9 form confirming authorization to work in the United States. ODHS will use E-Verify to confirm that you are authorized to work in the United States.
The Department of Human Services is a drug free workplace. All applicants will be subject to drug testing for Narcotics, Methamphetamine, and other controlled substances.
Your driving record must meet the qualifications of the ODHS SACU Driving Policy and will be considered unacceptable for offenses, including, but not limited to any misdemeanor or felony traffic offense in the last 24 months.
If you are offered a position, you will attend a three-week paid in-person training at our central office in Salem and you must become certified during this New Employee Orientation in the following:
First Aid/CPR Certification - Requiring you to complete compressions in a kneeling position.
Oregon Intervention System Certification (OIS) - Requiring you to physically hold an individual for safety while maintaining an approved restraint on clients which may involve quick movements, changes in physical positioning, and verbal communication with coworkers.
