Alphatec Spine develops and sells implants, instruments, and integrated technologies for spine surgery, including devices used in fusion procedures and related navigation or imaging systems gained through acquisitions like SafeOp Surgical. During surgery, surgeons use these implants and instruments, often with imaging or navigation tools to plan and guide the procedure, while the implants support fusion and healing. It differentiates itself with a broad product portfolio and enhanced technology through strategic acquisitions, giving it capabilities in navigation and advanced spine solutions beyond a narrow line of implants. Its goal is to improve spinal surgery outcomes and expand access to effective spine-care solutions.

Alphatec delivers $214M revenue and 53% adjusted EBITDA growth despite biologics headwinds

Alphatec Holdings reported strong Q2 2026 results with total revenue of $214 million, up 15% year-over-year, and surgical revenue of $196 million, up 17%. The medical device company saw adjusted EBITDA rise 53% to $36 million, with margins expanding 420 basis points to 17%. The company added 24% more net new surgeons and posted 20% case volume growth. Gross margin improved 260 basis points to 72.5%, driven by better inventory efficiency and favourable product mix. Alphatec generated positive free cash flow of approximately $1 million in Q2 and raised full-year adjusted EBITDA guidance to $140 million from $134 million previously. However, revenue per case declined 2.7% due to case mix shifts and international growth. The company now expects full-year revenue per case to decline in the low single digits, citing slower biologics attachment improvement.

Law firm investigates Alphatec after shares plunge 32% on missed EOS revenue guidance

Kirby McInerney LLP is investigating potential securities law violations by Alphatec Holdings and its senior management following a sharp share price decline. On 5 May 2026, Alphatec reported first quarter revenue of $192 million and disclosed that EOS revenue, from its proprietary imaging platform, was $14 million—below internal expectations. The company reduced its full-year 2026 EOS revenue guidance to approximately $77 million from $85 million, citing execution issues with equipment installations. Management stated that "installation timing was a challenge" and the company failed to fulfill committed units. Following the announcement, Alphatec shares fell 32%, from $10.23 to $6.99 per share. No lawsuit has been filed yet, and the investigation is ongoing to determine whether claims may be brought under federal securities laws.
