  • A high school diploma or equivalent is required.
  • The ability to pass criminal history background checks and monthly exclusion-list checks is required.
  • A valid driver's license is required.
  • Personal automobile insurance is required, with at least $300,000 in bodily injury liability coverage and $100,000 in property damage coverage.
  • The ability to communicate effectively in written and verbal formats is required.
  • A pre-employment drug screen is required.
  • The ability to bend, stoop, push, and pull on an ongoing basis is required.
  • The ability to lift and transfer clients using a two-person lifting technique may be required.
  • A flexible schedule, including possible overtime, weekends, evenings, and overnight awake shifts, is required.
  • The ability to work in residential treatment and community environments, including potentially uneven terrain and crisis situations, is required.
Responsibilities
  • Follow the organization’s policies, procedures, mission, and values.
  • Maintain client confidentiality in the facility and community.
  • Use collaborative problem-solving approaches when interacting with clients and colleagues.
  • Actively supervise and interact with clients in the community and facility.
  • Orient, train, and supervise residents to function safely and independently according to their personal care plans.
  • Provide services and support according to each client’s individual service plan.
  • Transport clients to and from appointments and activities and provide direct supervision as necessary.
  • Administer prescribed medications according to physician orders and agency policies and procedures.
  • Assist clients with making and keeping medical appointments and filling prescriptions.
  • Assist clients with food purchases, preparation, and cleanup.
  • Assist clients with housekeeping and maintaining home cleanliness.
  • Provide skills training and coaching in independent living, self-sufficiency, education, medical-system navigation, community living, and related areas.
  • Report suspected client-rights violations or abuse according to mandatory-reporting policies and regulations.
  • Transfer residents between wheelchairs, beds, chairs, and toilets, and push clients in wheelchairs when required.
  • Assist residents with eating, toileting, bathing, personal hygiene, grooming, cleaning, community access, spending, socialization, recreation, skill acquisition, and self-expression.
  • Participate in inventory, ordering, and purchasing of client program supplies, including food and medications.
  • Assess and manage crisis situations.
  • Use verbal intervention skills to de-escalate clients.
  • Identify risk situations and manage aggressive behaviors.
  • Maintain clean, sanitary, and safe conditions and report hazards.
  • Observe clients for behavioral changes and report them to the clinical supervisor.
  • Understand and follow safety procedures, fire drills, and evacuation plans, and conduct drills as assigned.
  • Participate in seclusion and restraint activities when necessary.
  • Maintain required training.
  • Document critical incidents using incident reports.
  • Document residents’ daily activities using PCS notes, contact logs, and activities-of-daily-living sheets.
  • Conduct or facilitate groups oriented to treatment needs.
  • Work with qualified mental health associates to meet facility, staff, and resident needs.
  • Provide documentation required by regulations and organizational policies.
  • Perform work safely according to established policies, procedures, infection-control requirements, hand-washing practices, glove-use requirements, and disposal procedures.
  • Cooperate with staff and demonstrate respect toward staff, residents, and visitors.
  • Contribute to a safe and well-maintained work environment.
  • Comply with federal, state, and agency health and safety reporting requirements.
  • Perform other duties as assigned.
