Position Summary                                                                        

The Occupational Therapist and Community Program Leader is responsible for planning, implementing, and evaluating community-based occupational therapy services that promote healthy aging, independence, and fall prevention for older adults. This position provides evidence-informed fall prevention programming, functional assessments, home safety education, and wellness interventions while collaborating with community organizations throughout Central New York. Occupational Therapist and Community Program Leader assists with program development, participant outcome measurement, data management, and community outreach to support CARE's mission of improving the health, safety, and quality of life of older adults.

This is a part time position working 25 hours per week.

This position is funded by external sources and is contingent on the continued receival of those funds.

Principal Duties and Essential Functions

Job duties specific to this position:

Occupational Therapy and Fall Prevention

• Conduct standardized assessments to identify fall risk, balance deficits, mobility limitations, and functional performance among older adults.

• Implement CARE's evidence-informed fall prevention and healthy aging programs in community settings.

• Develop individualized recommendations and intervention strategies to improve balance, strength, mobility, confidence, and safe participation in daily activities.

• Provide education on fall prevention, home safety, aging in place, adaptive strategies, and healthy aging to participants, caregivers, and community organizations.

• Conduct home safety evaluations and provide recommendations for environmental modifications and adaptive equipment when appropriate.

• Monitor participant progress and modify interventions as needed to maximize outcomes.

• Collaborate with healthcare professionals, senior service agencies, community organizations, and residential facilities to coordinate services and promote participant success.

• Assist with supervision and mentoring of occupational therapy students during fieldwork experiences, as assigned.

Community Programming and Program Development

• Deliver group education sessions and evidence-informed exercise components that support CARE's healthy aging and fall prevention initiatives.

• Assist in the development, implementation, and evaluation of new occupational therapy and healthy aging programs.

• Collaborate with CARE staff to expand programming and strengthen partnerships throughout the community.

• Develop educational materials, participant resources, and program content that support healthy aging and fall prevention initiatives.

• Participate in community outreach events, health fairs, workshops, and presentations representing CARE and Le Moyne College.

Data Management and Program Evaluation

• Collect, maintain, and analyze participant outcome data from CARE's fall prevention programs, ensuring accurate and timely entry into the CAMS reporting system.

• Prepare and submit monthly reports summarizing program outcomes, participation metrics, and performance data for the Onondaga County Office for Aging and other stakeholders.

• Monitor data quality and reporting compliance within the CAMS reporting system, utilizing findings to support program evaluation, continuous quality improvement, and grant reporting.

• Analyze program outcome data to identify trends, measure program effectiveness, and support strategic planning and funding initiatives.

• Develop and deliver annual presentations highlighting program outcomes, community impact, and key performance metrics to county partners, funding organizations, and prospective stakeholders.

Qualifications

• Graduate degree in Occupational Therapy from an accredited program.

• Current New York State Occupational Therapy license in good standing.

• Experience working with older adults, community-based practice, fall prevention, health promotion, or wellness programming preferred.

• Experience conducting standardized assessments and interpreting participant outcomes preferred.

• Excellent organizational, project management, and computer skills.

• Excellent group facilitation, presentation, and interpersonal communication skills.

• Comfort with basic digital tools for content sharing and creation (e.g., Canva, Instagram, Facebook, Zoom, Microsoft Office, etc.).

• Ability to collect, analyze, and report program outcome data using electronic reporting systems, including the CAMS reporting system.

• Experience preparing reports and presenting program outcomes to community partners, stakeholders, and funding organizations preferred.

• Ability to establish and maintain collaborative relationships with participants, caregivers, community partners, healthcare professionals, and students.

• Ability to work independently while contributing effectively as part of an interdisciplinary team.

• Enthusiasm for CARE's mission of promoting healthy aging, fall prevention, and community-based wellness.

• Must possess a reliable vehicle for travel throughout Central New York.

Additional Information:

Pay Range: $38.00 - $40.00 per hour
