Requirements
  • A high school diploma or GED is required.
  • Two or more years of experience in social service delivery with elderly people, people with disabilities, and/or family populations is required.
  • Demonstrated working knowledge of supportive services and other resources in the area served by the project is required.
  • Demonstrated ability to advocate, organize, solve problems, and provide results for participants served is required.
  • Good communication, comprehension, and interpersonal skills are required.
  • Working knowledge of Windows, Microsoft Office Suite, and the World Wide Web is required, demonstrated by a passing score on a computer literacy test.
  • The ability to speak, read, write, and understand English is required.
  • The ability to apply common-sense understanding to carry out detailed instructions is required.
  • The ability to perform basic arithmetic operations, including calculations involving money, using a calculator or Excel spreadsheet, is required.
Responsibilities
  • Coordinate programs and activities for eligible individuals on a group basis.
  • Serve as a liaison to community agencies, network with community service providers, seek out new services, identify low-cost providers, and negotiate discounts when appropriate.
  • Monitor community-agency service provision, document participant use in the designated documentation management system, and manage supportive services where appropriate.
  • Develop and maintain a resource directory of state and local service providers.
  • Organize educational events concerning health care, agency support, life skills, and referral sources, and link eligible participants to those events.
  • Identify partners to provide health services and screenings to participants at home.
  • Assist participants in building informal support networks with other participants, peers, family, and friends.
  • Engage volunteers in the community and/or property when appropriate under volunteer policies and procedures.
  • Coordinate individual participant services through a person-centered, non-clinical assessment, intervention, and monitoring process.
  • Conduct outreach to eligible individuals, provide program education, offer enrollment, engage participants to identify needs, and make referrals to community agencies.
  • Conduct telephone and/or in-person participant assessments and screenings according to program social and health requirements and established time frames.
  • Collaborate and communicate appropriate information with care and service colleagues to achieve participant goals.
  • Provide customer service by answering questions, addressing concerns, and assisting with basic needs from a person-centered perspective.
  • Assist participants in acquiring and using community services addressing social determinants of health, including housekeeping, meals, transportation, personal services, financial assistance, adult day care, counseling, and other requested services.
  • Follow required monitoring schedules, including telephone and/or in-person visits in participants' homes as directed by protocols or needs.
  • Gather reports, participate in interdisciplinary meetings, and assist with grants or special programs benefiting participants.
  • Build rapport with participants while maintaining appropriate professional boundaries.
  • Manage time independently, complete required tasks and documentation within identified time frames, and work autonomously and as part of a team.
  • Respond to phone, email, fax, and other communications in a timely and professional manner.
  • Record labor time accurately according to program and employer policies.
  • Complete timely electronic and written documentation of individual interactions, assessments, and services.
  • Complete all required forms for participants using the Service Coordination program.
  • Complete care plans and monitor them with participants, including following up on referrals and services.
  • Submit documentation needed for funder and quality performance reports.
  • Complete annual education requirements, including compliance, privacy, security, and Health Insurance Portability and Accountability Act training.
  • Report complaints, abuse allegations, misappropriation of funds or property, and other compliance concerns to appropriate supervisors, while advocating participants' preferences and self-determination.
