Behavioral Health Care Manager + Provider Success role:

The Role

The Behavioral Health Care Manager + Provider Success professional is a core member of Cerula Care’s collaborative care team. This is a full-time, Colorado-based role designed to test and build the value of an on-the-ground presence with our oncology partner.

The role is approximately 80% behavioral health care management and 20% provider success and sales support. The successful candidate will deliver high-quality, relationship-centered care to patients while building trusted, productive relationships with oncology providers, nurses, advanced practice providers, and practice teams across oncology practice locations.

This person will work remotely for core clinical and administrative responsibilities while traveling regularly to oncology offices throughout Colorado for site launches, in-person trainings, provider and clinical team visits, and ongoing relationship development.

Key Responsibilities

Clinical Care Management (approximately 80%)

• Conduct initial biopsychosocial intakes, including safety screening and behavioral health assessments such as the PHQ-9 and GAD-7.
• Provide follow-up visits and asynchronous support using brief, evidence-based interventions, including Behavioral Activation, Motivational Interviewing, mindfulness, psychoeducation, and coping skills development.
• Build strong, trusted relationships with patients and engage them effectively through telehealth and, when appropriate, in-person interactions.
• Support patients in identifying barriers to care, increasing engagement, building self-management skills, and implementing behavioral health treatment recommendations.
• Lead weekly interdisciplinary case review meetings with the Consulting Psychiatrist and relevant care team members, ensuring high-risk members are discussed and psychiatric recommendations are documented.
• Coordinate closely with Cerula Care’s behavioral health team, including the Consulting Psychiatrist, coaches, patient engagement team, and operational partners.
• Document member progress accurately and promptly in Cerula Care’s HIPAA-compliant electronic medical record and client registry.
• Identify patients who are not improving or may need a higher level of mental health care, and communicate concerns to the medical provider and Consulting Psychiatrist.
• Facilitate referrals for clinically indicated services outside Cerula Care under the supervision of the BHCM Lead.

Provider Success and Field Engagement (approximately 20%)

• Serve as a visible, trusted Cerula Care partner to providers and practice teams in and around Denver and Colorado Springs areas.
• Attend site launches and provide in-person training and education for providers, nurses, advanced practice providers, and office teams.
• Build excellent provider relationships through consistent follow-up, responsiveness, and a strong understanding of each site’s workflow and referral needs.
• Help providers and staff understand Cerula Care’s services, appropriate patient populations, referral pathways, and the role of the BHCM within the Collaborative Care Model.
• Act as a liaison between referring specialists, practice teams, patients, and Cerula Care, ensuring clinical and administrative questions are answered or routed promptly.
• Partner with Cerula Care’s commercial and customer success leaders to support referral engagement, identify barriers and opportunities, and strengthen adoption across practice sites.
• Gather and communicate field feedback to improve launch execution, training, provider experience, patient experience, and referral workflows.
• Track relationship-building and field activities and contribute to practical, outcomes-focused plans for improving provider engagement and appropriate patient referrals.

Education, Certification, and Experience Requirements

Required

• Must hold an active, unrestricted CO state license in counseling or social work
• Master’s degree in Counseling, Clinical Mental Health Counseling, Social Work, Marriage and Family Therapy, Psychology, or a closely related behavioral health field.
• At least three years of clinical care management experience, including experience working with people with cancer, serious illness, chronic medical conditions, or other complex health concerns.
• Experience conducting behavioral health intakes, psychosocial assessments, safety screens, treatment planning, and timely clinical documentation.
• Experience using evidence-based interventions such as Motivational Interviewing, Cognitive Behavioral Therapy-informed techniques, Behavioral Activation, psychoeducation, and coping skills development.
• Demonstrated ability to build strong relationships with both patients and healthcare providers.
• Strong presentation and facilitation skills, including comfort conducting in-person trainings and clinical team visits.
• Residence in Colorado and ability to travel regularly to locations throughout the state.
• Valid driver’s license and reliable transportation for in-state travel.

Preferred

• Experience in oncology, integrated care, collaborative care, behavioral health care management, primary care, palliative care, or telehealth.
• Experience delivering brief, goal-oriented interventions within a measurement-based care model.
• Experience in provider success, clinical outreach, practice engagement, healthcare partnerships, implementation, or sales support.
• Knowledge of oncology practice workflows and experience working directly with physicians, nurses, advanced practice providers, and multi-site clinical organizations.
• Fluency in Spanish 

Location and Travel Requirements

This is a Colorado-based hybrid and field role. The employee will work remotely for clinical and administrative responsibilities and will travel regularly to oncology offices in and around Denver Colorado.

In-person responsibilities will include site launches, trainings, provider and clinical team visits, relationship development, and other practice-facing activities. Travel frequency may vary with launch and engagement needs.

Schedule: Primarily aligned with Colorado business hours, with flexibility for patient care, site launches, and practice needs.
