Requirements
  • Graduate of an accredited Physical Therapy program with a Bachelor's, Master's, or DPT degree.
  • Hold an active California Physical Therapist license.
  • Hold CPR/BLS certification.
  • Have reliable transportation and be willing to travel to patients’ homes.
  • Be comfortable working independently in a home health setting.
  • Have strong communication and organizational skills.
Responsibilities
  • Provide in-home physical therapy evaluations and treatments.
  • Assess patients’ strength, mobility, balance, and ability to function safely at home.
  • Create and update individualized plans of care that support independence and recovery.
  • Help patients regain mobility and confidence in their home environment.
  • Educate patients and caregivers on exercises, safety, and fall prevention.
  • Document visits and evaluations in WellSky/Kinnser in a timely manner.
  • Collaborate with nurses, therapists, and the broader care team to support patient outcomes.
  • Supervise Physical Therapy Assistants and support their clinical development.
  • Participate in care planning, case conferences, and discharge planning.
Desired Qualifications
  • Home health experience.
  • Experience charting in Kinnser/WellSky.
