Requirements
  • High School Diploma/GED
  • Bachelor’s Degree in related field and 1 year of related experience or 2 years of related experience
  • Must hold a valid driver’s license
  • Must have access to an operational and insured vehicle and be willing to use it to transport consumers
  • Must be able to perform each essential duty satisfactorily
  • Must have ability to communicate effectively in writing
  • Must be able to read English and communicate effectively in the primary language of the program to which he/she is assigned
  • Must be able to multi-task with frequently changing priorities
  • Must possess basic computer knowledge
  • Must have a demonstrated ability to work independently and as part of a larger resource team
Responsibilities
  • Collaborate with the Team Leader to provide continuous quality improvement of individualized programs and services through the use of both internal and community resources
  • Assist team members with the development of program curriculum designed to promote individual growth and self determination
  • Develop and implement comprehensive written therapeutic service plans/individual support plans and implementation process for all assigned consumers
  • Convey changes in consumer goals to the larger internal support team
  • Maintain member records as required including timely and thorough completion of necessary program assessments, progress reports and other required documentation
  • Compile review summaries for therapeutic service plans/individual support plans

  • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations
