- Must be a Registered Nurse currently licensed by the state and in good standing.
- Previous experience in a hospital, long-term care facility, or other healthcare-related facility is required.
- Prior experience completing Minimum Data Set assessments and care plans is required.
- Must be knowledgeable of general, rehabilitative, and restorative nursing and medical practices and the laws, regulations, and guidelines governing long-term care.
- Participate in the Patient Assessment Process by completing items on the Minimum Data Set and accompanying Care Area Assessments.
- Assist in developing individualized patient care plans and identifying patient needs and projected outcomes as required by federal and state regulations.
- Contribute to the nursing center interdisciplinary Resident Assessment Instrument process while following guidelines for completion, computer encoding, and electronic transmission of the Minimum Data Set.
- Identify changes in patient condition with the interdisciplinary team and coordinate the scheduling and completion of indicated assessments.
- Assist with Activities of Daily Living training for new hires and provide ongoing continuing education to existing staff.
- Attend educational programs to maintain knowledge of current Resident Assessment Instrument and Prospective Payment System regulations.
