- A Master's degree in social work from a school of social work accredited by the Council on Social Work Education is required.
- An active Ohio social work license is required.
- A valid driver's license and reliable transportation insured in accordance with Ohio requirements are required.
- At least one year of experience in a health care field is required, including experience working with patients and families coping with terminal illness and dying.
- Effective communication, comprehension, documentation, interpersonal, and computer skills are required.
- The ability to assess patient needs and formulate individualized patient care plans is required.
- The ability to work in a field setting and make sound judgments is required.
- Conduct an initial comprehensive psychosocial and bereavement risk assessment of each patient, family, and caregiver within five calendar days after hospice election.
- Obtain and make referrals to community resources, including financial assistance, and provide appropriate follow-up.
- Assist families and caregivers with placement of patients in facilities for basic care, respite, or inpatient care when appropriate.
- Provide psychosocial and anticipatory grief counseling regarding the impact and implications of terminal disease and patient care.
- Assess patients' and families' psychosocial status and capacity to cope with disease, terminal illness, environment, and death; provide counseling and assist with community resources; and communicate findings to the team.
- Participate in developing plans of care and treatment and hold case conferences as needed.
- Prepare social histories and evaluations and plan interventions based on findings.
- Update, maintain, and submit accurate and timely clinical documentation according to agency guidelines and assist with discharge planning.
- Attend and participate in interdisciplinary team meetings to coordinate care plans, follow up on changes, solve problems, identify and assess social problems, evaluate family strengths and weaknesses, and discuss alternative interventions.
- Help team members understand significant social and emotional factors affecting patients and families.
- Provide care respectfully and protect patient dignity; report complaints, abuse allegations, misappropriation of patient property, and other corporate compliance issues.
- Mentor other personnel through orientation, field training, and in-service education for contractors and referral sources.
- Teach patients, families, and caregivers about disease progression and self-care techniques and provide counseling support or instructions as ordered by the physician.
- Hospice or home care experience is preferred but not required.
