Requirements
  • A minimum of 2 years of experience as a Registered Nurse.
  • A minimum of 2 years of experience in utilization management, case management, chronic care management, discharge planning, transitions of care management, cost/quality management programs, or a related field.
  • Availability to work during assigned clinic business hours.
  • Current working knowledge of chronic care management, discharge planning, utilization management, case management, performance improvement, or managed care reimbursement.
  • Competency in chronic care management, pre-acute and post-acute venues of care, and post-acute community resources.
  • Effective oral and written communication skills.
  • Strong technical skills, including data analysis and management, Microsoft Office Suite, and Electronic Medical Records.
  • Strong organizational and time management skills, with the ability to prioritize multiple tasks and role components.
  • Ability to work independently and exercise sound judgment when interacting with providers, payers, patients, and their families.
  • An active Texas, Arizona, or Multi-State Compact Registered Nurse license.
  • Ability to work in the center full time.
Responsibilities
  • Oversee chronic care and transitions-of-care management for high-risk patients within assigned care teams and neighborhood centers.
  • Serve as a resource to the multidisciplinary team for managing complex patients, including chronic care management assessments and care plans.
  • Perform telephone triage for patients, address issues appropriately, and forward messages for further intervention when needed.
  • Ensure efficient and organized patient transitions from acute and post-acute settings to home or another transitional care facility.
  • Perform comprehensive assessments of physical, mental, and social risk factors and identify and address barriers to care.
  • Collaborate with medical, nursing, and ancillary staff to eliminate barriers to efficient care delivery in the appropriate setting.
  • Coordinate and facilitate patient progression throughout the continuum of care.
  • Collaborate with physicians and multidisciplinary team members to facilitate care for designated patients; monitor progress; intervene as needed; facilitate diagnostic testing, treatment and discharge plans, modifications to the plan of care, appropriate levels of care, communication with the care team, and required documentation.
  • Coordinate and communicate with providers and care team members regarding discharge plans to ensure participation and readiness.
  • Ensure that plan-of-care elements, including discharge plans, are communicated to patients, families, and healthcare team members and documented as necessary.
  • Apply the Four Elements of the Coleman Model.
  • Coordinate post-discharge needs, including durable medical equipment, home health services, medications, and other supplies.
  • Proactively identify and resolve issues that impede diagnostic testing, treatment progress, or discharge.
  • Schedule patients for follow-up with a primary care provider or specialist within 7 days of discharge.
  • Reconcile discharge medications and work with the primary care provider and clinical pharmacist on post-discharge review.
  • Review and evaluate patients to determine whether they meet criteria for home health admission or admission to other transitional care institutions.
  • Track and monitor readmissions to acute care facilities and assist with rehospitalization-reduction initiatives.
  • Work with the clinical team to establish care programs that help prevent readmissions and hospitalizations.
  • Obtain patient medical records from acute care facilities, including orders, referrals, care team documentation, diagnostic testing results, and acute care visit summaries.
  • Use advanced conflict-resolution skills to ensure timely resolution of issues.
  • Identify at-risk populations using approved screening tools and follow established reporting procedures.
  • Refer cases and issues to the clinical leadership team and follow up as indicated.
  • Refer appropriate cases for social-work intervention as needed.
  • Collaborate and communicate with external case managers; initiate and facilitate referrals for home health care, hospice, medical equipment, and supplies.
  • Participate in clinical performance-improvement activities.
  • Use data to drive decisions and plan and implement performance-improvement strategies related to case management for assigned patients, including fiscal, clinical, and patient-satisfaction data.
  • Collect, analyze, and address variances from the plan of care with the multidisciplinary care team.
  • Document assessments, telephone calls, and patient interactions in the Electronic Medical Record in a timely manner.
  • Maintain professional growth and development by meeting mandatory and continuing-education and skills-competency requirements.
  • Perform other duties within the nurse’s scope of practice as assigned.

Nearly one-third of Arizona's population - approximately 2.3 million people - identify as Hispanic, according to the U.S. Census Bureau.
