Customer Service
  • Clinical Social Worker License required
  • Master's Degree of Social Work required
  • Relevant state licensure or registration
  • 5+ years' experience in the delivery of care to patients with CKD and ESkD or other chronic disease management preferred
  • Minimum of 2 years of experience in health care customer service or experience with Nephrology patient care in the physician office, dialysis center or other medical environment
  • Experience providing care through telehealth or other technology-enabled platforms preferred
  • Responsible for driving the IWH culture through values and customer service standards
  • Accountable for outstanding customer service to all external and internal customers
  • Develops and maintains effective relationships through effective and timely communication
  • Takes initiative and action to respond, resolve and follow up regarding customer service issues with all customers in a timely manner
  • Communicates and coordinates with patients and appropriate members of the patient's health care team in developing, executing, and reviewing results of care coordination efforts including the individualized plan of care
  • Manages referrals regarding patients at high risk of poor health outcomes, barriers to complete treatment recommendations, and/or with complex psychosocial barriers. Identifies root cause(s) of barrier(s), develops plan of intervention with an evidence-based approach, executes intervention with team members, and monitors success of intervention. Re-evaluates interventions and plans as necessary
  • Reports the discovery of unreported medical or social conditions or changes at home that may lead to adverse outcomes to the clinical team and ensures that these are referred to appropriate sources for attention
  • Maintains and updates clinical documentation using appropriate software to manage required information and data
  • Evaluates program processes and makes recommendations to management that will improve the effectiveness and efficiency of the program
  • Empowers patients and families by providing ongoing education regarding psychosocial issues related to kidney disease and all available support services
  • Educates patients and supports the completion of advance directives, POLST, DNR, and other EOL forms/information and wishes
  • Provides social service and community resource navigation and referral, including emergency food, housing, shelter, transportation, and healthcare insurance
  • Identifies appropriate community social service agencies and other resources to assist patients and families. This may include but is not limited to assisting patients in securing financial assistance, obtaining medical transportation, identifying mental health treatment and any other resources deemed necessary
  • Complies with all departmental required data collection and auditing activities, participates in quality improvement activities and project-based work
  • Reviews and complies with the Code of Business Conduct and all applicable company policies and procedures, local, state and federal laws and regulations
  • Completes other duties as assigned
