- Provide case management and care coordination.
- Accurately document observations, interventions, and evaluations related to client care management and services provided using a tablet.
- A minimum of one year of clinical experience is preferred.
- Knowledge of OASIS is highly preferred.
- Knowledge of the HCHB electronic medical record system is desired.
