Requirements
  • Current Licensed Practical Nurse license issued by the state in which services will be provided or current multi-state Licensed Practical Nurse license through the enhanced Nurse Licensure Compact (eNLC)
  • Completion of an accredited LPN Program
  • State criminal background check and Federal (if applicable), as required for regulated areas
  • Certification in Basic Life Support within 30 days of hire date
  • Must possess a valid and current driver’s license
  • One (1) year of clinical nursing experience
Responsibilities
  • Supports communication and coordination of discharge planning to ensure timely care for new patient referrals amongst pharmacy staff, home care nursing staff, care management, providers, and other clinical support staff
  • Assists in the development and monitoring of workflows, policies, procedures, projects, and compliance within home infusion services under supervision
  • Facilitates assessment of home infusion related issues, providing basic interventions or referrals as appropriate and within scope of practice
  • Demonstrates knowledge of specialty & core therapies and the home infusion referral, as well as payor qualifying criteria relevant to assigned responsibilities
  • Reports complaints and issues to supervisors/management as applicable for further evaluation and resolution
  • Maintains positive relationships with referral sources, care management, pharmacy staff, and home care nursing staff
  • Supports implementation and delivery of patient care plans and home infusion clinical programs, ensuring
  • appropriate follow-up and delivery of care as per the established plan
  • Accurately documents the coordination & transition of clinical care and patient education efforts following established protocols
  • Participates in training and cross-training initiatives to promote team efficiency and productivity
  • Provides consultation and patient/caregiver education as needed to ensure care coordination among the care team
  • Supports identification, development, and monitoring of performance and quality improvement initiatives
  • Assists with obtaining prior authorizations for patient services, working with healthcare providers and insurance companies as needed
Desired Qualifications
  • Specialized LPN experience
  • Two (2) years of experience related to intravenous administration of medications
  • Two (2) years of Home Health experience
  • One (1) year experience in care management
  • Sales and Customer Service experience
