Requirements
- Must be a graduate of an approved school of nursing.
- Must have a minimum Bachelor's Degree in Nursing or three years of experience in long-term care.
- Must hold current New York State licensure as a Registered Professional Nurse or Licensed Practical Nurse.
- Must have a minimum of three years of experience in a supervisory capacity in a hospital, long-term care facility, or other related medical facility, including training in rehabilitative and restorative nursing practice.
- Must have a sincere desire to work with elderly, chronically ill, and disabled residents.
- Must have thorough knowledge of the medical, social, and psychological needs of residents.
- Must utilize good judgment in emergency situations.
- Must communicate effectively verbally with residents, families, and staff.
- Must be able to handle emergencies with calm and intelligent response.
- Must have computer literacy.
Responsibilities
- Provide professional supervision, guidance, and direction to staff on clinical and personnel issues in the long-term-care unit.
- Coordinate resident care in accordance with New York State Department of Health regulations.
- Provide support to residents and families in addressing clinical issues and challenges.
- Ensure staff adherence to safety and infection-control guidelines.
- Assist in establishing daily resident-care operations, including bathing, toileting, meals, medication administration, and activities.
- Communicate pertinent information to the interdisciplinary team to ensure resident and family needs are met.
- Perform daily rounds of all residents on the unit.
- Perform required resident assessments, including admission, elopement, and pain assessments.
- Develop a plan of care for each resident and document and communicate it on Resident Information Sheets.
- Document clinical observations and assessments for assigned residents in accordance with regulations, report findings to the designated Nursing Supervisor, and document the effectiveness of each plan of care.
- Prepare resident assignments for Certified Nursing Assistants based on resident needs and personnel competency.
- Communicate ongoing resident, family, and facility plans, concerns, and information to staff.
- Organize, oversee, and supervise assigned unit activities and staff assignments.
- Provide each Certified Nursing Assistant with an assignment and communicate pertinent resident-care needs.
- Monitor and evaluate unit activity flow and patient care delivered by Certified Nursing Assistants, including turning and positioning, mealtimes, and bathing.
- Ensure Certified Nursing Assistant documentation, including activities of daily living, bowel movements, and meal intake, is completed.
- Communicate pertinent resident findings to interdisciplinary team members.
- Assist other staff members in completing their duties.
- Orient newly hired nursing personnel as assigned.
- Perform other related duties as required and assigned.
