Must be a licensed driver and have access to an insured vehicle if transporting clients
Responsibilities
Attend the initial meeting with family to develop client’s Daily Living Skills Plan
Meet with the assigned clients and their families weekly to provide skills training and education on areas of improvement identified during the initial assessment
Understand and implement the Daily Living Skills Plan
Document each session with assigned client as required by established documentation standards and submit documentation in a timely and consistent manner
Develop an understanding of your client’s abilities, development, strengths, and needs
Build and maintain rapport with clients and families
Addressing behavioral challenges by managing disruptive behaviors that may arise during skill development and implementing strategies to promote positive interactions
Present yourself in a professional manner when representing Northwest Community Partners
Complete all required initial and ongoing trainings as required
Maintain client confidentiality
Communicate clearly in oral and written form
Other duties as assigned
