Requirements
  • Current Registered Nurse licensure in good standing in the state or states of practice.
  • Ability to collaborate daily with nurses, social workers, chaplains, volunteers, and certified nursing assistant leadership in a team environment.
  • Ability to work one-on-one with patients while receiving support from the patient care team.
  • Ability to work independently with phone, email, and written communication; meetings with healthcare professionals; and hospice rules and regulations.
  • Time management skills to manage visits, documentation, meetings, scheduled visits, required meetings, urgent needs, and multiple competing concerns through prioritization and triage.
  • Ability to learn an electronic medical records system.
  • Willingness to drive with reliable transportation, a valid driver's license, and auto insurance to travel across the service area.
  • Availability to work Tuesday through Saturday from 8:00 a.m. to 4:30 p.m.
Responsibilities
  • Provide care and services to a group of hospice patients in their residences, including private homes, skilled nursing facilities, and assisted living facilities.
  • Serve as a contributing member of the interdisciplinary team by providing expertise regarding psychosocial assessments, interventions, and community resources.
  • Work in a case management, admissions, or resource nursing role.
