Requirements
  • A Master's Degree in Physical Therapy is required.
  • A current state-issued professional license is required.
  • A valid state driver's license is required.
  • Current CPR certification is required.
  • Pediatric clinical experience is required.
  • Home health experience is required.
  • Ability to read and write simple correspondence and speak effectively in small groups is required.
  • Ability to interpret instructions and solve practical problems involving a variety of concrete variables is required.
  • Proficiency with Microsoft Office Suite, record keeping, and routine database activity is required.
  • Ability to manage multiple tasks simultaneously is required.
  • Ability to perform the physical demands of the role, including standing, walking, reaching, sitting, kneeling, crawling, and strenuous therapy activities, is required.
  • Ability to lift a minimum of twenty-five pounds is required.
  • Close-vision ability is required.
Responsibilities
  • Evaluate, organize, and conduct medically prescribed physical therapy for pediatric patients in the home health setting.
  • Direct patients and caregivers in selected tasks to restore, reinforce, or enhance performance.
  • Examine, evaluate, and administer remedial treatments for pediatric patients.
  • Provide screenings, evaluations, direct treatment, family and caregiver training, counseling, home programming, and discharge planning.
  • Coordinate patient care with office staff and other disciplines.
  • Communicate with pediatric patients and families regarding progress, problems, home programs, and other therapeutic-process issues.
  • Refer patients and family members to appropriate professionals, agencies, services, and resources.
  • Consult with physicians regarding patient progress and maintain appropriate records.
  • Maintain supplies and equipment.
  • Perform internal and external public education functions, including periodic in-service presentations when needed.
  • Participate in the Quality Assurance and Performance Improvement program.
  • Attend continuing education activities and expand the scope of service.
  • Maintain an adequate caseload with appropriate productivity.
  • Participate in team efforts to achieve company and departmental goals.
  • Collaborate with others by sharing knowledge and skills.
  • Mentor students and new hires.
  • Participate in professional organizations and activities.
  • Use initiative and independent judgment in performing job responsibilities.
  • Serve as a backup service provider for other caseloads as needed.
  • Comply with company policies, practices, and procedures.
  • Perform other duties as assigned.

Provides in-home healthcare for children up to 18 with special needs, offering nursing, speech, physical, and occupational therapy, plus case management. A team of licensed clinicians visits the child’s home to deliver individualized therapies and nursing care, while case managers coordinate services across disciplines for comprehensive, continuous care. The focus on pediatric home health with a multidisciplinary, in-home approach and ethical, quality care sets it apart from broader healthcare providers. The goal is to improve health outcomes and daily function for children with special needs while supporting families through high-quality, home-based care.
