Requirements
  • Strong communication, interpersonal, and organizational skills.
  • Excellent data analysis skills.
  • Excellent computer skills, including Microsoft Office, Word, and Excel.
  • Broad general knowledge of medical terminology.
  • A Bachelor's Degree or higher is required, preferably in a medical care field.
  • At least one year of experience working in a health care setting.
  • The ability to operate phones, computers, and other office equipment with sufficient manual dexterity.
  • The physical ability to kneel, bend, and perform light lifting.
  • The ability to write and speak clearly using the English language and hear at normal speaking levels in person and by telephone.
  • The ability to perform close vision, depth perception, and focus adjustment tasks.
  • The ability to travel as required to work with staff.
Responsibilities
  • Perform clinical auditing and data abstraction of electronic health records and quality improvement data.
  • Perform data entry for key-metric reporting to internal and external stakeholders and submit accurate, complete, and timely data through quality data-reporting systems.
  • Identify and close gaps in care prospectively and retrospectively through pre-visit planning and communication with patients, providers, and other care-team members.
  • Track labs and imaging as part of pre-visit planning.
  • Use population health tools to conduct outreach to patients with gaps in care.
  • Participate in data validation of internal and external clinical quality reports.
  • Assist with performance improvement, population health, and telehealth projects as directed.
Desired Qualifications
  • Demonstrated knowledge of electronic health record systems.
  • Experience with care coordination, quality management initiatives, and tracking.
  • At least one year of experience in a care-related quality role.
  • Bilingual Spanish-speaking ability.
