Requirements
  • Registration as a Licensed Practical Nurse with CLHA in good standing.
  • A minimum of 3-5 years of experience as a practicing nurse.
  • A valid driver's license and access to reliable transportation.
  • A current, clear Vulnerable Sector Screening.
  • Comprehensive patient health assessment skills.
  • An understanding of the principles of community nursing practice.
  • Proven experience leading and managing a team.
  • Knowledge of clinical documentation standards and practice guidelines.
  • Organizational, interpersonal, and teaching skills.
  • Commitment to professional development in clinical practice relevant to home health care.
  • Critical thinking and problem-solving skills.
Responsibilities
  • Supervise and support Health Care Aides to ensure practices comply with legislated standards of practice and company policies and procedures.
  • Respond to and advise on or resolve concerns from community staff, patients and families, and other team members.
  • Provide ongoing leadership through regular huddles, face-to-face meetings, and one-on-ones.
  • Initiate patient safety incident investigations and documentation to ensure timelines and action plans are met.
  • Collaborate with nursing team members to support clinical documentation and submission requirements according to specific funder reporting guidelines.
  • Engage patients in problem-solving using a collaborative approach, provide health teaching, and build rapport and trust to identify patient health care needs, strengths, capacities, and goals.
  • Engage staff in problem-solving by offering education, coaching, and mentoring to resolve problems.
  • Maintain the knowledge, skills, and attitudes required to provide safe, efficient, and effective service by keeping current with changes in the practice setting that affect scope of practice.
