Requirements
  • Bachelor degree (BSN or BS) preferred
  • Licensed Registered Nurse in good standing for the State of Michigan
  • Two to five years of experience in nursing with adult and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing, or hospital based medical-surgical setting
  • Ability to utilize word processing, spreadsheet, presentation programs, databases, and other software relevant to the job
  • Demonstrates customer focused interpersonal skills to interact in an effective manner with practitioners, patients with diverse opinions, values, and religious and cultural ideals
  • Demonstrates ability to work autonomously and be directly accountable for practices

Responsibilities
  • Supports BMG ambulatory primary care and specialty practices through triaging calls and coordinating care as needed
  • Collaborates with the primary care practices, specialists, and other members of the health care team, and patients/families to ensure the delivery of quality, efficient, and cost-effective health care services
  • Follows evidence-based clinical guidelines, preventive guidelines, and protocols in care delivery to the patient
  • Coordinates care across acute and other ambulatory settings and helps patients/families understand health care options
  • Connects patients with Acute Care Managers and other resources as needed
  • Regularly communicates both orally by telephone, and in writing with patients, physicians, and other healthcare professionals to exchange information pertinent to their information provided
  • Demonstrates effective problem-solving and critical thinking in care for patients
Desired Qualifications
  • Experience in ambulatory care is desirable experience as well as acute care or post-acute care setting
