Requirements
  • Possess a current license to practice as a registered nurse in the State of Virginia.
  • Have at least two years of work experience as a professional nurse, preferably including at least one year in hospice care.
  • Be able to assess and respond to the needs of patients and loved ones or caregivers in varied settings.
  • Be skilled in nursing practice and able to cope with loved ones’ or caregivers’ emotional stress while remaining tolerant of individual lifestyles.
  • Be able to make meaningful observations, perform skilled assessments, and write comprehensive, accurate reports.
  • Have a valid driver’s license, a good driving record, use of a reliable vehicle, and the ability to drive year-round to locations.
  • Be able to take direction, work independently, and work effectively as part of a multidisciplinary team in a positive, professional manner that supports the organization’s mission.
Responsibilities
  • Accept an assigned patient caseload from the Patient Care Coordinator or designee.
  • Assume primary responsibility for a patient caseload, including the assessing, planning, implementing, and evaluating phases of the nursing process.
  • Initiate communication with attending physicians, hospice staff members, and other agencies as needed to coordinate optimal care and use of resources for patients and their loved ones or caregivers.
  • Maintain regular communication with the attending physician concerning patients and their loved ones or caregivers.
  • Seek input from other team members regarding the patient plan of care to obtain additional knowledge and support.
  • Provide accurate and timely documentation in electronic medical records.
  • Participate in and contribute appropriate information to interdisciplinary group meetings.
  • Coordinate the physical care of patients by teaching primary caregivers, volunteers, and employed caregivers or by providing direct care as appropriate.
  • Meet regularly with hospice nursing staff to review problems or unique issues from caseloads, share professional support, and exchange feedback aimed at enhancing professional growth.
  • Attend patient care conferences and facilitate discussion of caseload issues for full staff discussion, consultation, and evaluation.
  • Supervise and document the NAC every two weeks or as assigned.
  • Inform the Patient Care Coordinator of unusual or potentially problematic patient or family issues.
  • Serve as the associate nurse when responding to patients or loved ones or caregivers of other primary nurses during off-hours or when requested by the Patient Care Coordinator to meet patient or family needs during the absence of other primary nurses.
  • Provide appropriate support at the time of death and during bereavement.
  • Participate in hospice orientation and in-service training programs for professional staff.
  • Participate in agency and community programs as required to promote professional growth and understanding of hospice care.
  • Maintain familiarity with agency policies and the rules and regulations of state and federal bodies governing hospice.
  • Accept other assignments as appropriate.
