Diverge Health provides in-home, personalized support for patients by deploying trained community health teams to work with primary care practices. It partners with risk-bearing practices to extend care beyond the clinic, focusing on Medicaid patients and addressing social determinants of health. The company recruits, trains, and deploys health coaches and provides the coordination infrastructure needed to manage patient populations across several states. Unlike purely software-driven platforms, Diverge Health emphasizes hands-on care delivery and care coordination to improve outcomes and lower costs for its partner providers. The overarching goal is to help primary care practices improve patient health, reduce expenses, and advance health equity.
